INSPECTION REPORT 255/88022-01 ADDITIONAL INFOFJfATION NRC Inspection Report 50-255/89032 indicated that violation 255/88022-01:
Inadequate quality verification resulting in rriisplugged or defective tube plugs,*was remaining open until the licensee provided the proper documentation for NRC review regarding the A and B Steam Generator cold leg plugged tube verification.
In order to clarify and close this issue Consumers Power Company is providing the following information and explanation.
The original commitment regarding assurance tha.t no anomalies exist from past tube plugging operations in _either the A or B steam generator comes from Consumers Power Company letter dated February 16, 1989, "Response to Notice of Violation Presented in Inspection Report 255-88022, Corrective Actions to be Taken to Avoid Further Non-Compliance and Date When Fuel Compliance Will Be Achieved."
We stated, "To assure that no other anomalies exist from past tube
~lugging operations, ~xisting video ~apes of the tube sheets will be reviewed.
If these tapes do not provide a reliable review, a 100% tube sheet verification via video camera will be performed in both A and B steam generators.
The tube sheet verification will be compJeted during the next opportunity when the steam generators are opened or during the next refueling outage if deemed necessary.
This outage is currently planned to occur in 1990."
Consumers Power Company moved this commitment up to the 1989 Maintenance Outage and performed a tube sheet scan on A and B steam generator hot legs.
This examination disclosed no discrepancies for misplugged tubes.
The projected time to perform this verification was 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> where as the actual time spent was 2 days.
This represented a significant increase in time, money and man-rem exposure.
A cold leg entry was not planned for the 1989 Maintenance Outage.

and 1987 plugged tubes were reviewed and all tubes plugged during those outages were verified as correct except for 16 tubes in the A steam generators and 12 tubes in the B steam generator which could not be confirmed due to insufficient video.
Consumers Power Company made the decision not to perform a 100% video verification of the A and B steam generators cold legs because of the following:
- a. no -mi_splugged tubes were identified in the hot leg scans.
- b. no discrepancies were noted in the review of the video tapes of the 1983, 1985, 1987 plugged tubes in the cold legs
- c. the close proximity of the Palisades steam generator replacement
- d. the large man-rem exposure required versus the additionql assurance gained The cold leg plug verification documentation for 1983, 1985 and 1987 outages is available through the Palisades NRC Resident Inspector.
