Specifically, Consumers Power Company was required t.6 review rec"eipt inspection "requirements and internal controls for fasteners and to independently determine, through testing, whether fasteners in stores meet mechanical and chemical specific~tion requirements.

Our response for Palisades is provided herein.

Item 1 Describe a) the characteristics currently examined during receipt inspection of fasteners (i.e., head markings for grade and manufacturer symbols, review of certified material test report or certificate of conformance), and b) internal controls utilized during storage and issuance from stock to assure the appropriate use of fasteners.

Response

a)

Receipt Inspection Receipt inspection is done only on "Q" procurements, therefore, characteristics other than quantity or shipping damage are not checked on "non-Q" receipts.

For "Q" procurements, characteristics inspected are those which are specified on the purchase order, such as dimensions, required documentation or packaging.

Dimensional inspection would normally verify length, diameter and threads per inch.

Required documentation would usually include a certificate of conformance, but would rarely require a material test report.

In addition to what the purchase order specifies, markings are verified to match the ASTM, ASME or SAE specifications of the purchase order.

Storage and Issue 2

Both "Q" and "non-Q" fasteners are stored in essentially identical environ-ments but are physically separated per ANSI requirements.

All fasteners are stored in a dry, heated warehouse.

They are not stored by size or dimension.

When they have a single application (control rod drive, feed pump casing, etc), they may be stored with the other basic component parts to which they apply.

The storage location (shelf, cabinet, drawer) is indexed by assigned stock numbers.

Fasteners are not individually wrapped, rather, they are packaged (usually in a clear plastic bag) by purchase order number.

For "Q" fasteners, the packages are tagged to identify the purchase order number; for "non-Q" packages they are not generally tagged.

There is a third category of fast;ener and that is a "non-Q" general use or bulk supply.

These fasteners are not assigned stock numbers and are stored in bins or trays by size.

These are hardware store variety fasteners.

"Q" and "non-Q" items with stock numbers are issued in accordance with the similar controls.

That is, they are issued to a work document governing and detailing the work being done.

"Q" items are not issued in bulk; "non-Q" items may be if requested.

For "Q" issued material, trac.eability exists from the purchase order to the item end use. or from the end use back to the purchase -order.

For the "non-Q" ~aterial, with stock numbers, traceability only exists to the stock number and not to a specific purchase order.

For "non-Q" fasteners without stock numbers, there are minimal controls.

They are not issued with issue tickets, there are no records of end use and bulk issue for shop use or shop supplies is permitted.

There is no traceability from purchase order to use or any other material control requirements imposed on their issue.

Item 2 Select a minimum sample of ten (10) non-safety related fasteners (studs, bolts, and/or cap screws), and ten (10) safety-related fasteners (studs, bolts, and/or cap screws) from current, in use, stock.

The sample is to be obtained by the licensee with the participation of an NRC inspector.

Fasteners procured to meet the following chemical and mechanical properties are of interest:

A-193 grades B7, B8, and B16; SAE J429 grades 5 and 8; A-449; A-325 Types 1, 2 or 3; A-354 grades BB, BC, BD; A-490; A-320 LTM; A-307; A-563; or equivalent.

Response

The samples were obtained in late November with the participation of the 3

senior site resident inspector, taking into account those fasteners of interest.

Refer to Attachment 1 which contains the fastener testing data sheets and which contains vendor data, mechanical, and chemical test data.

Pages 1 to 2A of Attachment 2 are safety-related samples; pages 3 and 3A are non-safety related samples.

Item 3 For the selected sample of fasteners in item 2, include a sample of typical nuts that would be used with each fastener (one-for-one).

In particular, nuts purchased to the chemical and mechanical specifications of A-194 are of interest.

Response

Typical nuts were obtained with the participation of the senior site resident inspector; using the same selection criteria as that for the fasteners (see ).

Item 4 Chemical testing shall be performed on all samples.

Mechanical testing shall be performed on each safety-related fastener.

Hardness testing shall be performed on each nut and non-safety-related fastener.

All testing shall be performed by a laboratory which the licensee has qualified for this type of testing and appears on the licensee's approved vendor list. Testing performed shall be done in accordance with the requirements of the fastener's specifi-cation, grade, and class, and the test shall evaluate the ultimate tensi.le strength, hardness and chemical properties as required by the fastener's specification, grade, and class.

Each sample shall be tagged with the sample's ID number.

Response

Chemical, mechanical, and hardness testing results are provided in.

Testing was performed in accordance with the requirements of the samples' specifications.

Mechanical and hardness testing was performed by Consumers Power Company Laboratory Services Dept.

Chemical testing was performed by Chicago Spectra Service Laboratory of Chicago, IL.

Both testing laboratories are qualified by Consumers Power Company.

The result.s of all tests, together with supporting information, are to be reported to the NRC utilizing the format shown in Attachments 1 and 2 of this bulletin.

Include the names and addresses of suppliers and manufacturers of safety-related fasteners and, to the extent possible, of non-safety-related fasteners.

For any fastener found out of specification, provide an evaluation of the safety significance including consideration of the most limiting application.

Response

The results of the tests conducted are included in Attachment 2.

Three fasteners (PAL-10, -5, and -43) were found to be out-of-specification. All three fasteners had hardness values out-of-specification.

One safety-related stud, sample PAL-10, had a Rockwell C hardness value of approximately 41 HRC as compared to the accepted range of values of 24 to. 37 HRC.

Although this hardness value is slightly out of range, the results of the mechanical testing showed the ultimate tensile strength, yield strength, percent elongation and reduction area percent to be within the minimum values.

Also the results of the chemical anl~ysis showed the stud to be in specification.

We, therefore, find this stud to be of acceptable quality and *conclude there is no safety signifi~ance associated with its use or other studs from the same batch.

Only one other stud.procurred with sample PAL-10 is presently. in use and it is in a non-safety related application.

The two non-safety related bolts, samples PAL-5 and PAL-43, had Rockwell C hardness values which were slightly lower than the minimum required value of 25 HRC.

The test values of 24.6 and 23.9, respectively, are the average of five tests.

Although no mechanical testing was conducted, the chemical testing for these bolts showed them to meet the specification requirements.

From ASTM A-370, it can be seen that the approximate tensile strength for 24 HRC is 119 ksi.

This nearly equals the required minimum tensile require-ments of 120 ksi for the bolt specifications.

Although only an approximation, this does provide additional assurance of the bolts' strength.

Since these bolts are used in non-safety related applications, there is no safety signifi-cance in the use of these bolts or others from the same batches.

Item 6 Based on the results of the testing and review of current procedures, describe any further actions being taken to assure that fasteners used in the plant meet the requisite specifications and requirements and that the operability of safety-related plant components is not affected.

Response

5 Testing results did not identify any signific~nt anomalies.

Deviations from specifications are slight and most likely attributed to natural variations in chemistry, heat treatment, and testing. Results did not indicate that any of fastener components were mis-marked or found to be traceable to lesser strength specifications.

No further actions were determined to be necessary.

Following NRC evaluation of the responses from all licensees, Consumers Power Company requests that they be informed of any suppliers or manufactures which evidence poor performance so that we may evaluate their acceptability for future procurements.
