REVISED RESPONSE TO IE BULLETIN 85-03 Kenneth W Berry Director Nuclear Licensing Consumers Power Company's letter of July 31, 1987 transmitted a response to the NRC's June 26, 1987 request for additional information regarding IE Bulletin 85-03, "Motor Operated Valve Common Mode Failures During Plant Transients Due To Improper Switch Settings".

In our response, we committed to revising the listing of High Pressure Safety Injection (HPSI) motor operated valves (MOVs)contained in our original May 15, 1986 response to IE Bulletin 85-03 to include unlisted MOVs, M0-3041, 3045, 3049 and 3052, and to include the revised list within an update to our May 15, 1986 correspondence.

Accordingly, this transmittal supersedes our May 15, 1986 submittal and, in conjunction with our July 31, 1987 correspondence, comprises our response to IE Bulletin 85-03.

Consumers Power Company had committed to providing this u*pdate by September 15, 1987 but due to delays was unable to do so.

We informed the Region III Reactor Projects Section 2A Chief and were granted a two-day extension to our commitment date.

Below are action items a thru e from IE Bulletin 85-03 along with our updated responses.

ITEM a Review and document the design basis for the operation of each valve.

This documentation should include the maximum differential pressure expected during both opening and closing the valve for both normal and abnormal events to the extent that these valve operations and events are included in the existing, approved design basis, (ie, the design basis documented in pertinent licensee submittals such as FSAR analyses and fully approved operating and emergency procedures, etc).

When determining the maximum differential pressure, those single equipment failures and inadvertent equipment operations (such as inadvertent valve closures or openings) that are within the Plant design basis should be assumed.

Nuclear Regulatory Commission Palisades Plant Revised Response to ~EB 85-03 September 17, 1987

RESPONSE

. 2 Consumers Power Company (CP) has compiled the data contained in Attachment A to this submittal.

The present motor operated valve (MOV) switch settings for the Palisades Plant were confirmed by Limitorque in 1984, in an effort to enhance information and practices related to setting torque switches.

We are in the process of reconfirming the adequacy of these switch settings. This work will be completed by the upcoming October 1987 Maintenance Outage.

Maximum differential pressures listed in Attachment A were determined by Combustion Engineering (CE) or CP and verified by CP.

In determining the maximum differential pressure, considerations of various system design basis functions were postulated both upstream and downstream of the valve as shown in Attachment A.

ITEM b Using the results from Item a above, establish the correct switch settings.

This shall include a program to review and revise, as necessary, the methods for selecting and setting all switches (ie, torque bypass, position limit, overload) for each valve operation (opening and closing).

If the licensee determines that a valve is inoperable, the licensee shall also make an appropriate justification for continued operation in accordance with the applicable technical specification.

RESPONSE

Thrust calculations were performed using the maximum differential pressure, valve and operator data from field walkdowns and valve and operator data supplied by the vendor.

This effort verified that all operators are adequately sized and capable bf opening/shutting the valves during maximum differential pressure conditions.

Based upon these thrust calculations, a target thrust window was established for each operator's torque switch settings. Sirice September 12, 1985 all torque switch settings have been documented on a controlled drawing.

All changes to switch settings (ie, torque switch, position limit, torque bypass, etc) are evaluated and controlled via our specification change process.

See item c for methods of selecting and setting switches.

ITEM c Individual valve settings shall be changed, as appropriate, to those established in Item b above.

Whether the valve setting is changed or not, the valve will be demonstrated to be operable by testing the valve at the maximum differential pressure determined in Item a above with the exception that testing motor operated valves under conditions simulating a break in the line containing the valve is not required. Otherwise, justification should be prov~ded for any cases where testing with the maximum differential pressure cannot practicably be performed.

This justification should include the alternative to maximum differential pressure testing which will be used to verify the correct settings.

Note:

This Bulletin is not intended to establish a requirement for valve testing for the condition simulating a break in the line containing the' valve.

However, to the extent that such valve operation is relied upon in the design basis, a break in the line containing the valve should be considered in the analyses prescribed in Items a and b above.

The resulting switch settings for pipe break conditions should be verified, to the extent practical, by the same methods that would be.used to verify other settings (if any) that are not tested at the maximum differential pressure.*

RESPONSE

Consumers Power has selected Movats, Inc., to perform diagnostic testing of our operators.

"As-found" diagnostic testing of each motor operated valve will be performed to demonstrate the "as-found" operability of the operator.

Following the "as-found" diagnostic test, any needed valve maintenance or valve operator maintenance will be performed by Babcox & Wilcox Field Services Division, using CP approved procedures.

Upon completion of valve and/or valve operator maintenance, the torque switch and limit switch on each operator will be set using the Movats diagnostic equipment.

As mentioned previously, the target thrust windows will be used to establish the "as-left" switch settings.

Current plans are to test as many valves as practical that are identified in IE Bulletin 85-03 at the maximum differential pressure.

However, if Plant conditions prohibit the differential pressure test, the "Movats data base for valve operation against a differential pressure" will be substituted for the performance of these tests.

ITEM d Prepare or revise procedures to ensure that correct switch settings are determined and maintained throughout the life of the Plant.

Ensure that applicable industry recommendations are considered in the preparation of these procedures.

RESPONSE

Babcock & Wilcox has reviewed, revised and rewritten all of Palisades Plant's Limitorque maintenance procedures to ensure that switch settings are properly set and maintained.

Babcock & Wilcox had been chosen to review and revise our Limitorque procedures because they are an authorized Limitorque repair facility and are adequately prepared to address.all applicable industry recommendations concerning Limitorque operators.

These maintenance procedures have been reviewed and approved by Palisades Plant personnel and will be used

. to perform all future Limitorque motor operator maintenance.

Industry information from equipment vendors INPO (SOERs and SERs), NSSS vendors (CE ADP Info Bulletins), and the NRC is reviewed and evaluated through the Palisades operating experience review program.

The program is administered by the Plant Safety Engineering (PSE) group with recommendations resulting from the review of applicable items forwarded to the appropriate plant department(s) for action as necessary.

Nuclear Regulatory Commission Palisades Plant Revised Response to IEB 85-03 September 17, 1987 participates in the NPRDS program in which equipment failures can be tracked.

Maintenance procedures will be revised as necessary to incorporated the PSE recommendations resulting from the industry information.

Periodic reviews (a minimum of every two years) by knowledgeable personnel also ensure the maintenance procedures contain up-to-date information.

ITEM e Within 180 days *of* the date of this Bulle.tin, submit a written report to the NRC that:

(1) reports the results of Item A and; (2) contains the program to accomplish Items B through D above including a schedule for completion of these items.

RESPONSE

The maintenance outage in which IE Bulletin 85-03 program actions will be completed is scheduled to begin in October 1987.

Final documentation verifying completion of the IE Bulletin 85-03 program at the Palisades Plant will be submitted to the NRC within 60 days of the program's completion, but not later than January 15, 1988.
