RESPONSE TO INSPECTION REPORT 87-006 NRC Inspection Report 87-006 dated April 30, 1987 transmitted one violation which requires a response by May 27, 1987. The following is our response to that item.

Violation (255/87-006-01) 10 CFR 50 Appendix B Criterion XVI "Corrective.Action" requires in part that, "in case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition". Palisades' Administrative Procedure No 3.03, "Corrective Action", implements this requirement to take corrective action to preclude repetition in Articles 4.7, 6.3, 6.4j 6.5 and 6.6.

Contrary to the above, on November 8, 1985, a low pressure safety injection pump and a safety injection tank were concurrently inoperable and the corrective actions described in the Licensee's January 22, 1986 and April 11, 1986 letters were not adequate to preclude repetition.

  • A second similar event involving a concurrently inoperable diesel generator and component cooling water pump took place on March 10, 1987.

Corrective Actions Taken and Results Achieved The subject event of November 8, 1985 was attributable to human err9r. On March 10, 1987 the Limiting Condition of Operation was entered due to an equipment problem with component cooling water pump, P-52C, subsequent to the removal of diesel generator 1-2, for normal pre-planned maintenance. Although the.Limiting Condition of Operation (Technical Specification 3.7.2) for both pieces of equipment being out of service was not exceeded, the time in the Limiting Condition of Operation was prolonged by the failure to identify and document the. Nuclear Regulatory Commission 2 Palisades Plant Response to IEIR 87-006 May 27, 1987 corrective action taken for the November 8, 1985 event did not preclude recurrence, this second event was subjected to an independent review performed by the Human Performance Evaluation System (HPES) Coordinator. This independent review revealed that the event was indeed prolonged as the result of human error. The root cause of the event was identified to be a late log entry regarding the inoperability of diesel generator 1-2 due to post-maintenance testing conducted on diesel generator 1-2. Consequently, when component cooling water pump P-52C was removed from service for repair, the on-duty Shift Supervisor did not recognize that the safety related equipment were concurrently inoperable.

Corrective Action To Be Taken To Avoid Further Noncompliance A memo will be issued to Operations Department personnel and additional discussion will be provided in Operations Department Continuing Training reiterating the necessity for proper and timely attention given to the Limiting Condition of Operation Status Board when concurrent safety related equipment is out of service.

Incr~ased management attention is being directed toward deficiencies resulting from inattention to detail. Use of the HPES Coordinator to provide an independent review of events, including human error, will enhance the Plant's ability to identify and correct deficiencies.
