RESPONSE TO IE INSPECTION REPORT 86-018 Kenneth W Berry Director Nuclear Licensing IE Inspection Report 86-018 dated August 8, 1986 transmitted one violation which requires a response by September 8, 1986.
The following is our response to that item.
Violation (255/86018-03)
Technical Specification 6.8.1.a requires that written procedures shall be established, implemented, and maintained for applicable procedures in Appendix "A" of Regulatory Guide 1.33 endorsed by CPC-2A which includes procedures governing log entries.
Administrative Procedure 4.01, "Shift Operations," Paragraph 5.7.2.b.4 re-quires that the Shift Supervisor's Log contain notifications and reportable occurrences.
Contrary to the above, on June 17, 1986, the Shift Supervisor's Log did not contain an entry for the diesel generators (DG) actuation which is a report-able occurrence or for the 10CFR50.72 notification of same.
Also, no entry was made for the 10CFR50.72 notification concerning the DGs actuation of June 10, 1986.
Response
Corrective Actions Taken and Results Achieved The occurrence was caused by a lack of attention to detail by the involved Shift Supervisors.
Accordingly, the need to maintain complete logs has been emphasized to all Shift Supervisors.
Shift Supervisor log entries are being monitored by the Palisades Operations Superintendent, with feedback given to OC0986-0142-NL02 8610100045 860908 PDR ADOCK 05000255 G

Corrective Action To Be Taken To Avoid Further Noncompliance 2
Although the specific reportable events listed in the Notice of Violation were not logged in the Shift Supervisor's log, they were logged in the Shift Engineer's log.
Prior to September 1, 1986, the Shift Engineer has functioned as Site Emergency Director (SED), and was therefore the most cognizant indi-vidual regarding reportability situations.
As of September 1, 1986, the Shift Supervisor will function as the SED, thereby better facilitating the logging of reportable events in the Shift Supervisor log.
