RESPONSE TO IE INSPECTION RERPOT 86-002 Kenneth W Berry Director Nuclear Licensing One item of violation was identified in IE Inspection Report 86-002 dated March 14, 1986.
The following is our response to that item.
Item 1:
Violation 255/86002-04 Technical Specification 6.12.1 states that any individual or group of individuals permitted to enter a high radiation area shall be provided with or accompanied by one or more of the following:
a radiation monitoring device which continuously indicates the radiation dose rate in the area; a radiation monitoring device which continuously integrates the radiation dose rate in the area and alarms when a preset integrated dose is received; or_ an individual qualified in radiation protection procedures who is equipped with a radiation dose rate monitoring device.
This individual shall be responsible for providing positive control over the activities within the area.
Contrary to the above, on January 21, 1986, a worker was permitted to work in a high radiation area, the purification filter room, without being provided with a referenced radiation monitoring device and without having his activities within the area under positive control by an individual qualified in radiation protection procedures.
Corrective Action Taken And The Results Achieved The occurrence resulted from the failure of the contract Health Physics (HP) technician who was covering the work to fully understand his responsibility to provide control over all activity within the high radiation area.
Consequently, a worker was left unattended for approximately 30 minutes after OC0486-0069-NLOlo -8604250021 8604 ---

Palisades Plant Response to IEIR 86-002 April 11, 1986 the work in the high radiation area was physically completed, while the worker was performing fire watch duty.
All HP technicians have been briefed regarding this incident and their responsibilities with respect to the requirements of Palisades Technical Specification 6.12.1.
Corrective Action To Be Taken To Avoid Further Violation The error by the HP technician was caused by a lack of sufficient guidance in department procedures and training regarding the requirements for monitoring work in high radiation areas.
Appropriate guidance will be added to the site specific HP training lesson plans by June 1, 1986, to ensure that this information is adequately communicated to contract HP technicians.
Department procedures will be revised to clarify the HP technician's responsibilities with respect to this requirement by July 1, 1986.
