Item - Infraction Technical Specifica~ion 6.b.l requires, in part; implementation of procedures "recommended in Appendix 'A' of Regulatory Guide 1.33 (November 1972)" which includes "procedures for the control of maintenance... and modification work
" Administrative Procedure 9.2, "Equipment Specification and Minor Field Changes" stipulates use of a Specification/Field Change Form (SFC) for documen-tation, review, and approval of minor field changes made during maintenance"
... to ensure that the quality of equipment is not degraded."
Contrary to the above, a modification (replacement of fuel oil filter drain plugs with valved drain taps on emergency diesel 1-1) was made during maintenance (MO 78~8072) on November 28, 1978, but the licensee failed to use an SFC Form.
Required reviews and approvals were not obtained.
The modified system subsequently failed on January 3, 1979, spraying the diesel with fuel oil and rendering it in-operable.
Response
Corrective Action Taken and Results Achieved At the time of the failure, the diesel generator was in operation.
The electrical loads were transferred from tl1e generator, and the engine was shut down.
The 1-2 diesel generator was tested to verify that it was operable.
Diesel generator 1-2 was inspected, and it was verified that the valved drain tap was not installed on

2 it. The valved drain tap was removed from the 1-1 diesel generator, thereby restoring that portion of the system to conformance with applicable drawings.
Corrective Action to Avoid Future Noncompliance Both diesel generators have been inspected to determine if there were any other deviations from the applicable drawings.
A total of five minor discrepancies were identified, and it has been concluded that all the changes were made prior to the implementation of the procedure which governs the "Equipment Specification and Minor Field Changes (SFCs)." Accordingly, it has been concluded that the most recent change to the diesel was an isolated case of nonadherence to the Administrative Procedures related to SFCs.
Administrative controls related to SFCs have been reviewed with all mechanical maintenance supervisors.
Date When Full Compliance Will be Achieved Three of the five noted changes have been analyzed and determined to be accept-able.
The remaining two will be analyzed and/or corrected by April 1, 1979.
