4. Apparent Cause~

Operator error - a valve was inadvertently left open at the end of the shift.

This val*e allowed water to flow to the neutralizer tan.~. The tank overrlowed and its contents at pH of 2.5 were released to the mixing basin at about 150 g_nm for approxi~..ately 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.
