This refers to the inspection
conducted
by C. A. Patterson
of this office on
January
15,
1994 - February
18,

inspection
included
a review of
activities authorized for your Browns Ferry facility.
At the conclusion of
the inspection,
the findings were discussed
with those
members of your staff
.
identified in the enclosed report.

Areas
examined during the inspection
are identified in 'the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as 'specified in the enclosed
Notice of
Violation concerning
performance of a containment
visual inspection
surveillance,
control of a switchyard modification, missed fire watch,
appendix
R design .errors,
and failure to make required notifications.
We are
concerned
about the first violation because
a
TS required surveillance
was not
properly completed
and not detected
during data reviews
by cognizant
, per'sonnel.
This failure created
the potential for not detecting
damaged
components within the primary containment.
We are concerned
about the second
violation because
of the potential for uncontrolled switchyard activities to
adversely affect plant operations.
We are concerned
about the missed fire
watch because
of continuing problems in the fire protection area.
We are
concerned
about the Appendix
R design errors
because
of the potential
For
a
plant fire to adversely affect safe
shutdown.
We are concerned
about the
failure to make required notifications because
of the conclusions
reached
by
the plant licensing staff.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.

control of a switchyard modification should address
why the site quality
organization,
modifications task monitoring programs,
and procedure for
'ontrol of contractors
did not identify this problem.
After reviewing your
response
to this Notice, including your proposed corrective actions
and the
results of future inspections,
the
NRC will determine whether further
NRC
enforcement
action is necessary
to ensure
compliance with NRC regulatory
requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed in the

Room.
