In the Matter of Tennessee Valley Authority Docket Nos. 50-259 50-260 50-296 BROWHS FERRY NUCLEAR PLANT (BFH) - REVISIOH TO TVA RESPONSE TO NOTICE OF VIOLATION (NOV) 92-29, WITHDRAWINQ A CORRECTIVE ACTION COMNITMEHT HRC issued Inspection Report 92-29 on September 11, 1992, which cited TVA with a violation for failure to follow procedures.
Specifically, the violation stated that Site Standard Practice (SSP) 12.3, Equipment Clearance Procedure, was not followed when a )ob supervisor failed to sign onto a clearance prior to beginning work.
TVA provided its response to the HOV on October 13, 1992.
TVA stated in its response that this= event was caused by a lack of attention to procedural i

TVA took prompt corrective actions to address this event, including personnel corrective actions, conducting supplemen a
ng t 1 traini for affected personnel holding clearance authorization, and adding additional con ro s o

to remove contractor personnel from the clearance list that had not held a clearance for more than 90'days.
TVA also coamitted to enh ance the work sign-on process by providing facilities at a single location for individuals involved in Unit 3 recovery activities to sign onto clearances.
TVA expected to complete this enhancement by December 17, 1992.

U.S. Nuclear Regulatory Commission DEf: i4 >9S2 performing daily valkdoms t'o monitor implementation of the assigned design changes, and verifying that appropriate clearances and vork permits have'een approved and issued.
Bach task manager is also responsible for ensuring that the contractori are in full compliance vith the clearance program.
Since implementation of this program, no additional contractor-related clearance procedure violations have, occurred.
Pp TVA considers 'that the actions discussed above provide a more effective
'recurrence control. 'Additional facilities*for. clearance and vork, sign-on authorizations for Unit 3 recovery activities are no longer considered necessary.
Therefore, TVA is vithdraving its commitment to enhance the vork sign-on process at BFH by adding facilities.
