BROHNS FERRY NUCLEAR PLANT (BFN) = UNIT 2 NELDING ISSUES ON CIVIL PROGRAMS During a recent visit to the BFN site to review closure of Employee Concerns, David Smith of your staff requested further discussion of Employee Concern Number 50-132-BFN-01. This employee concern was based on an inspection of welds which yielded a number of rejectable discontinuities. Also, prior to mid-1983, some support welds were not included in a documented quality inspection program. The corrective action plan for the employee concern was to verify weld attributes under the various Unit 2 civil restart programs.

Although a closure package is not yet finalized showing evidence of completion of this issue for all civil programs, necessary actions have been initiated.

The enclosure to this letter provides greater detail of the various programs implemented to resolve this Employee Concern at BFN Unit 2.

ENCLOSURE BRONNS FERRY UNIT 2 CIVIL PROGRAMS AND NELDING ISSUES The following civil programs are design complete:

Seismic qualification of cable tray supports Seismic qualification of conduit and conduit supports Seismic qualification of Heating, Ventilation, and Air Conditioning (HVAC)

The welding issues related to each of these programs have been addressed in the January 18, 1989 letter to the Nuclear Regulatory Commission (NRC) from R. L. Gridley, TVA Manager of Nuclear Licensing and Regulatory Affairs, titled Browns Ferry Nuclear Plant NRC Inspection Report No. 50-260/88-13, Response to Held Project Inspection Items. NRC acceptance is documented in Supplement 1 to the Safety Evaluation, Report (SER) for the Browns Ferry

- Nuclear Performance Plan NUREG - 1232, Volume 3 pages 3-2 through 3-12.

. The following civil programs are ongoing:

79-14 Program Piping Supports Small Bore Program Piping Supports Instrument Tubing Supports Miscellaneous Steel Outside of the Drywell Drywell Steel Control Rod Drive Insert and Hithdrawal Miscellaneous Steel Piping structural support welds at BFN are being evaluated during the current outage under the 79-14, Small Bore Piping, and Instrument Tubing Programs consistent with each program's boundary commitment for restart.

These programs include inspection, data collection, and evaluation of the following specific weld attributes; weld presence, type, size, length, location, and obvious gross weld defects. The NRC inspection of TVA's Small Bore Piping and Instrument Tubing Programs (reference 1) reviewed TVA's implementing procedures (reference 2) for collecting support weld data during configuration walkdowns and found the procedures acceptable. The walkdown procedure used for the small bore piping program is also being used for collecting weld data on the 79-14 program supports. Helds determined not to meet qualification compliance requirements are being modified under the individual programs. A description of each program's weld inspection and evaluation requirements are as given below.

79-14 Pro ram Pi in Su orts The 79-14 program for Unit 2 restart performed inspection, data collection, and evaluation of welds on piping supports within the boundaries of the program. Inspection and data collection of support configuration included the weld attributes listed above. Each support was evaluated based on the support inspection data and modifications were made as necessary.

Small Bore Pi in Pro ram Su orts The small bore piping program performed inspection and data collection for a representative sample of supports, which included the weld conditions identified above. Detailed evaluations of this support sample were performed to determine attributes which cause qualification concerns within the piping and supports. The remaining piping and support installations in the scope of the small bore piping program were then evaluated against these generic design attributes for qualification.

The results of these evaluations did not identify welding as a generic design attribute for support qualification. However, to ensure support integrity, the engineering walkdown reviewed for non-standard or damaged supports. As suspect supports are identified, detailed evaluations are performed, including weld qualification.

TVA commitment NCO 860326095 tracks this program to completion.

Instrument Tubin Pro ram Su orts The tubing program was performed using the same methodology as the small bore piping program, i.e , a representative sample was analyzed to determine

generic attributes. These generic attributes were then the basis for the evaluation of the remaining instrument tubing population. As in the small bore program, welding was not identified as a qualification attribute.

However, if the engineering walkdown identifies any non-standard or damaged supports, then they will be subject to a detailed evaluation, including weld qualification.

TVA commitment NCO 890128004 tracks this program to completion.

Miscellaneous Steel Outside of Dr well The structural welds on miscellaneous steel supporting the 79-14 program pipe supports received a 100 percent engineering walkdown utilizing requirements specified by Site Directors Standard Practice (SDSP) 9.8. SDSP 9 ' requires verification of weld size, length, and location which was used to perform the engineering evaluations. Modifications were issued as required to ensure structural integrity of the steel framing. The closure of Condition Adverse to Quality Report (CAQR) BFP890550 will document completion of this activity.

t Page 3 of 3 The structural welds on miscellaneous steel supporting the Long Term Torus Integrity Program (LTTIP) pipe supports were required to be verified by Note 3(I) of the general notes drawing 48B1005-1 for ECN P0361. This note required the field to verify existing welds as shown on the miscellaneous steel drawings. Note 4 required the field to contact NE for evaluation and resolution if an inspection parameter could not be met. Thus, the structural weld sizes and configurations utilized in the NE evaluations will be matched against the as-constructed welds. The closure of ECN P0361 will document the completion of this activity.

Dr well Steel The structural welds on the upper drywell platform steel were required to be verified by note 8 of the general notes drawing 488983-1 for ECN P0998. This note required the field to verify existing welds as shown on the platform miscellaneous steel drawings and modify/repair as required. Thus, the structural weld sizes and configurations utilized in the NE evaluations will be matched against the as-constructed welds. The closure of ECN P0998 will document the completion of this activity.

The structural welds on the lower drywell platform steel were inspected in accordance with the Special Mechanical Maintenance Instruction (SMMI) 1.1.E which predates SDSP 9.8. However, the inspection was upgraded to SDSP 9.8 requirements to resolve Corrective Action Report (CAR) BF 86-0199. Thus, the structural weld sizes and configurations utilized in the NE evaluations wi 1 1 be matched against the as-constructed welds. The closure of CAR-BF 86-0199 will document completion of this activity.

Control Rod Drive (CRD) Insert and Withdrawal Lines Su ort Frames The existing structural welds on the support frames supporting the CRD insert and withdrawal pipe supports are required to be verified by note 31 of the general notes drawing 47H2468-100-1 for ECN P0859. This note requires the field to verify existing welds as shown on the frame drawings and modify/repair as required. The closure of ECN P0859 will document the completion of this activity.
