LICENSEE EVENT REPORT NO. 2017-009, DEPARTMENT OF THE ARMY

Dear LTC Hannan:

This letter refers to your November 9, 2017, correspondence, in response to the reporting requirements of 10 CFR 20.2201 (b) for theft or loss of licensed material. Thank you for informing us of the incident of August 16 - 20, 2017, involving the loss of four tritiated exit signs (TES) as well as your corrective and preventive actions documented in your letter (EN 53021, NMED 170495).

Your letter indicated that you do not possess any other generally-licensed devices. A review of the NRG General Licensing Tracking System (GLTS) by our staff was only able to determine that only one TES was distributed to your facility by Emergi-Lite. The other three TES may not have been reported by the distributor, not entered into GLTS, or entered into GLTS incorrectly or in a manner not easily searched under in GLTS. For your records; the TES model number is WSLX-1061 R, the serial number is 435625, the initial activity was 7.5 Curies of tritium, and the date of distribution was January 4, 2008.

Our review of GLTS identified a number of generally-licensed chemical/explosives detectors that were distributed to your facility. During the telephone conversation between Charles Betoney II, of your staff, and Steven Courtemanche of this office, Mr. Betoney stated that the information on the detectors distributed by Rapiscan, Inc. (formerly Morpho Detection, LLC and GE Homeland Protection, Inc.) is still current. In addition, Mr. Betoney stated that the detectors distributed by Smiths Detection had been sent for disposal and that he would check and ensure that the reporting requirements of 10 CFR 31.5(c)(8) were met. Further, Mr. Betoney stated that, as an additional protective action, he or any future Installation Radiation Safety Officer would be informed of any decision to discontinue the use of any devices containing radioactive material.
