DSN 101 Oiffuser discharge structure - The chlorine injected into the raw cooling water and high pressure fire protection system exceeded the permitted daily maximum of 730 pounds.

The amount of chlorine added on November ll, 1987 was 1,978 pounds.

The chlorine (in the form of sodium hypochlorite) is injected for c'lam control and is diluted as it passes through the diffusers into the rivers.

Cause and eriod of noncom liance After initiating sodium hypochlorite injection by the plant surveillance instruction procedure a problem was encountered with one of the valves on the raw cooling water system.

The procedure was discontinued but the valves were not returned to their normal configuration resulting in improperly controlled sodium hypochlorite injection.

The period of noncompliance was 8 hours9.259259e-5 days 0.00222 hours 1.322751e-5 weeks 3.044e-6 months and 20 minutes.

Ste s taken to reduce eliminate and revent recurrance of the noncom 1 in discharcie The plant surveillance instruction will be revised to require Chemistry support for initial valve alignment and review of caution steps for initiation of sodium hypochlorite injection. It has been reemphasized that a

Site Director's Standard Practice requires that the system be returned to normal if problems are encountered with execution of a procedure.

Corrective action with regard to personnel error is still being evaluated.

8 2 Office Building Drain Oil and grease content exceeded the permitted daily maximum of 20 mg/1.

The oil and grease content of the discharge on November 19, 1987, was 46 mg/l.

Cause and eriod of noncom liance An oil leak from the control bay chiller 1-B was the cause.

The noncomplying sample was taken at 1015 hours0.0117 days 0.282 hours 0.00168 weeks 3.862075e-4 months on November 19, 1987.

A followup sample taken at 1312 hours0.0152 days 0.364 hours 0.00217 weeks 4.99216e-4 months had a concentration of 13.0 mg/1.

The return to compliance was verified with two additional samples of <5.0 mg/1 at 1507 and 1900 hours0.022 days 0.528 hours 0.00314 weeks 7.2295e-4 months .

A sample taken on November ll, 1987 was in compliance.

The maximum period of noncompliance" was 8 days, 3 hours3.472222e-5 days 8.333333e-4 hours 4.960317e-6 weeks 1.1415e-6 months , and 17 minutes.

A routine maintenance request was filed on October 10, 1987 to repair the leak.

Following an investigation of this noncompliance, an imnediate attention maintenance request was filed November 24, 1987, to have the oil -cleaned up and a

drip pan placed under the leak from the chiller.
