Dear Ms. Silva,
As requested in your letter dated December 22, 2017, {PART 71 QUALITY ASSURANCE PROGRAM APPROVAL BIENNIAL CHANGE REPORT).
Whereas Pharmalucence Inc. has sold off its IBL service business {as of August 5th, 2014) and transferred our client list to a new vendor MIKRON Inc, whom will now be solely responsible for the maintenance, upkeep and shipment of the CIS-US Inc. Model IBL-437C lrradiator.
Pharmalucence Inc. hereby requests that the agency terminate our Quality Assurance Program Approval {No. 0642), Docket Number 71-0642.
