White transmitting IE Inspection Report Nos. 50-259/86-08, 50-260/86-08, and 50-296/86-08 for our Browns Ferry Nuclear Plant which cited TVA with one Severity Level V Violation.

50-259/86>>08, 50-260/86-08, AND 50-296/86-08 JOHN A. OLSHINSKI'S LETTER TO S. A. WHITE DATED APRIL 16, 1986 Item 1

'10 CFR 50, Appendix B, Criterion XIII and the accepted Quality Assurance (QA)

Program (TVA Topical Report TVA-TR75-1A), Section 17.2.13, require that items in the gA program be stored to prevent damage or deterioration. The QA program commits to Regulatory Guide 1.38, Revisi.on 2, which endorses ANSI N45.2.2-1972, Packaging, Shipping, Receiving, Storage, and Handling of Items for Nuclear Power Plants. Section 6.4.2(1) of this standard requires that items in storage have all covers, caps, or plugs or other protective closures intact.

Contrary to the above, during a tour of the Engineering Change Notice (ECN) warehouse on March 5, 1986, the inspector identified that most stainless steel tubing (AMG 1384) and a solenoid valve (Contract Number 84PJ5-834065) were not protected as required. Further visual inspection also identified additional examples of items in storage that were not. protected as required.

This is a Severity Level V violation (Supplement, I).

1. Admission or Denial of the Alle ed Violation TVA admits the violation.

2. Reasons For the Violation A lack of attentiveness to protective coverings for every item in storage.

3. Corrective Ste s Which Have Been Taken and Results Achieved Employee meetings held at the Engineering Change Notice warehouse on March 13 and March 20, 1986 emphasized the mandatory requirement that.

protective covers be maintained on components, including piping, fittings, and valves, in accordance with ANSI N45.2.2-1972.

A walkdown of the entire warehouse has been completed and any deficiencies in protective coverings have been corrected. The central receiving office has been notified of the violation and will ensure that shipments are properly protected before being, placed in storage.

4. Corrective Ste s Which Will Be Taken to Avoid Further Violations A followup program has been developed that will require a designated individual to check components on a weekly basis for protective covers which may have inadvertently become unsecured due to such factors as material handling or weather conditions.
