This letter serves to provide Ze Comm'ss'on with a final report of a deficiency relating to defects in the workmanship of certa'n wiring connections on American Warm'ng and Ventilating, Xnc. (AWV) dampe s. The deficiency was originally reported under tne provisions of 10 CFR 21 in AWV's letter to the NRC dated May 6, 1980. PLA-485, dated May 19, '980, advised the Comme ssion Bat PPEL was eva'uat'ng the subject condition for reportability under 10 CFR 50.55(e) . The information conta'ned in this reoort is submitted pursuant to the provisions of 10 CFR 50.55'(e) .

The attachment to this lette contains a description of the p"oblem, 'ts cause, safety implications anc significance and the correct'-ve action taken to preclude recurrence.

Attachment to PLA-716 .May 1, 1981 BACKGROUND Defects in the workmanship of some w'ring connections on fifty eight (58)

AWV dampers Model Number 8700 were identif'ed:n AWV's 10 CFR 21 notification letter to the NRC dated May 6, 1980. The de ects were originally discove'"ed in similar AWV dampers 'nstalled at another nuc'ear generating facility.under construct'on. The AWV letter indicated that SSES damoers were also affected and Bechtel QC consecuently tagged the subject dampers under their nonconformance control system pending AWV corrective action instruct'ons.

DESCRIPTION OF DEFICIENCY

,The def'c'ency relates to improperly crimped r'ng terminal connec ors and the butt splice connectors on wir'ng within tne junction boxes of the BDID's purchased under specif'cation M-33'6A. Some of the wires could be pulled out of connectors by a light hand appl'd tensile force. The AWV crimping tool 'he used in manufacture die not make crimps consistently capable of meeting the 70 lb.

.referenced by Bechtel purchase speci ication M-3'36A paragraph 7.1. Bechtel source inspection activities at AWV did not require physical checkng of the w'res and connectors for adequate crimos. Only a visual inspection was required.

thus the 'mproperly crimped connectors were not d'scovered dur'ng Bech el activities. source'nspection SAFETY IMPLICATIONS The unction of the BDID's 's to protect divisional'zed electrical and mechanical systems from the effec"s of a high energy p'pe break outside of conta'nment. It is intended that such breaks will be perm'tted to disable only one division of an electrical or mechanical divisionalized system. The dampers are used o provide isolation for rooms such as the RHR piping room and the Reactor Core Isolation Cooling Pump room. Fa'lure of these, or similar dampers, from provid'ng such isolation could possibly affect the sa e operation of the nuclear power plant.

The deficiency necessitated extensive repa'" to establ'sh the adequacy of the damoers to perform their 'ntended safety function. A significant breakdown 'n the quality assu"ance program at Awv was concluded to have allowed this deficiency to occur and go undetected. Tnerefore, the cond't'on is considered reportable under the conditions of '0 CFR 50.55(e) .

CORRECTIVE ACTION TAKEN The crimping tool used at AWV has since been placed under thei" 'calibration program to prevent inadequate crimps 'from being made in the future. Adc'tionally, the Bechtel Procurement Supplier Quality Inspection instructions have been modified to include a snot-check examinat'on o" terminal boxes,fo" correct wi es and connectors and proper connections. This 'ncludes physical and v sual checking of the integr'ty.of crimped connectors prior to the'r release from the factory.

Attachment to PL'A-716 May 1, 1981 issued a repa'r procedure titled "Wiring Connector Replacement

'7 AW/ has Procedure or Single and Double Isolation Dampers." This procedure 'dentified all necessary connector replacements. All replacements h'ave been completed and 100% QC inspection of the repaired BDlD's electr'cal connectors has been performed. All work has been accepted as satisfactory by Bechtel QC thus resolving the subject deficiency at SSES.

PL NQA is scheduled to perform a verificat'on of this correct've action in an unannounced audit wnich will be ccmpl'eted by May 22, 1981.
