Engineer~~ note: although the QA Flyer mentioned all ~torque operators StB & P!C hav~mg this problan, the sMcing is only ~ilved ~m ruing stan type valves. ~~ does not pertain to Che type of motor operators that are mounted on the valve shaft by neans of bore and keyway on a spl~med adapter. Also, at this time we have not ordered S!C i>odels nor do we have any at the Jobsite.

As a result of this, Eechtel SOHO notified Che Jobsite (IZ$ 095528, RIC3865, dated 11/8/78) of the potential problan and recamerded that all Limitorque operated valves be checked for properly staked lock nuts.

ANALYSIS OF SAPK Y VPTZCA~EGNS At Bechtel ~0 request Che Jobsite inspected a sample of fM<teen (15) risi~

stem Limitorque motor operated valves and subnitted a list of the results.

A review of this list indicated that several of these valves were required for safe shut-down of Che plant. Therefore if sMdag is not done on these it is cossible valves, that are required for safe shutdown of the plant, that these valves could become inoperable. Eecause of this fact, we informed PP&L Chat CM~s was a reportable condition as described by 10CFR50.55(e) m~

our letter BLP-10630, dated Y~ch 20, 1979 (DC4 10150R).

1V. CORRECT'IE AC%ON T~

The cause of this problan is Chat the valve suppliers either improperly staked the stan lock nuts durir~ operator/valve assanbly or Chat stak.'Ln" was not performed. ~e following corrective action steps have been taken:

a. Eechtel SFEO I~ identified all suspect Limitorque motor operated ris~

stan valves usirg the Valve List Index Report for Susqueha~ Stan Wec-tric Station Units 1 and 2.

b. Eechtel ~O requested the Jobsite to inspect/stake these valves.

To date all Limitorque lfotor Operated valves, des~ated for ~~tallation or installed m~ Units 1 and 2 of SSE'S have bem inspected/staked (see Table 1), with the following exceptions:

a total af 26 nor. "Q" nntor operated valves have been des'.wted as spare valves. 'Ihese valves will be added to the 47 non-"Q" motor operated valves specified in the CorrLLtion Report.

CGNCLUSICH 5N 26 80 3. ]. o 8 2 4 As stated in this report, all suspect Limitorque n;otor operated valves, designated for irstallation or as spares, have been identified ard measures have been taken to insp ct/stake these val.ves. In addition, letters have been sent to the fallowing valve suppliers inforning them af the potential problem: Anchor Darlirg, Nalworth, Henry Vogt, Yarway, N.V.D. and Pacific Valves. use letters were to assure that all Limitorque ris~~ sten motor operated valves at the suppliers were staked as well as all future orders.

Additionally, we reauested that the Eechtel PSQR's were aware of the stakLng requiranents arxi would ir~ct alI. risirg stem motor operted valves for proper stakLtg. Wherefore, we feel that we have adequately addressed generic problem which will be fina~ed with the clcsecut af NCR 3424 and the Condi-tion Report, P-960-
