Dear Mr. Grier:
This supplements the above referenced correspondence and provides a final report regarding stem nut staking on Limitorque motor operated valves.
The report has been prepared by Bechtel Power Corporation and concurred with by PPQL.
All Limitorcjue motor operated valves installed in Units 1 a 2 have been inspected/staked.
Those valves that have not yet been installed are considered indeterminate and are being controlled via Bechtel nonconformance and condition reports.
Please advise should you reguire further information.

As a result of this, Bechtel SPADO notified Ch Jobsite (DC8 095528, P1C3865, dated'l/8/78) of the potentia1 problem and reccrmerded t?wt all Liad.torque operated valves be checked for prope.ly staked lock nuts.
GZCRIPTION OP HK~~C DurIrg the assenbly of Che va1ve/operator, the lock nut (which locks the stem nut M position) must be staked.
D'his is not done, it is possible for the lock nut to back out of the threaded?ole and allow the stem nut to move up the stan and render the valve inoperable.
III.
ANALYSIS OP SAF2, 7 PPLICA~IONS't Bechtel SFHO request Che Jobsite inspected a sample of fifteen (15) rising stan Limitorque motor operated valves and suhnitted a list of the results.
A review of this list indicated that several of these valves were required for safe shut-down of the plant.
Werefore if sMcLng is not done on these valves, that are required for safe shutdown of the plant, it is possible that these va1ves could become inopemble.
Because of this fact, we ~ormd PP&L that this was a reportable condition as described by 10CrW50.55(e)

our letter BLP-10630, dated Y~ 20, 1979 (DCf 10150R).
CORRECTI'JE A~ON TAKE The cause of this problan is that the valve suppliers either improperly stahmd the stan lock nuts durir~ operator/valve assanbly or that s~g was not performed.
We following corrective action steps have been taken:
a.
Bechtel SFHO has identified all suspect Llmitorque motor operated ris~~
stan valves usfrg the Valve Lf.st Index Report for Susquehanna Stan ELec-tric Station Units 1 and 2.
b Bechtel GPFO requested the Jobsite to inspect/stake these valves.
To date all Lfmitorque Motor Operated valves, des~ted for installation or installed
~m Units 1 and 2 of SSES have bem expected/staked (see Table 1), with the folio~ exceptions:

A total of 33 "Q" Listed motor operated valves renal to be inspected/
staked (See Table IZ).
'Ihis work is documented in NCR 3424 and will be canpleted prior to close-out of'his NCR.

A total af 47 non-"Q" Listed antor operated
- valves, designated for installation, ren"~ to be irspected/staked.
'Ihis work wiLL be acccxn-plished by the use af Condition Report No. P-960.
c.
In addition to the 80 valves that have not been inspected/staked to cate,.
a total af 26 non-"Q" aator operated valves have been designated as spare valves.
'Ihese valves will be added to the 47 non-"Q" motor operated valves specified in the Condition Report.
V.
CONCLUSICH NAY26'80 '.o824 As stated in this report, all susp ct IdmLtorque motor opera,ted valves, designated for installation or as spares, have been icentified ard measures have been taken to insp ct/stake these valves.
In addition, letters have been sent to the followirg valve suppliers informing them af the potential problem:
Anchor Darling, Vialworth, Henry Vogt, Yarway, N.V.D. and Pacific Valves.
Ihese letters were to assure that all Limitorque rising sten motor operated valves at the suppliers were staked as well as all future orcers.
Additionally, we recuested that the Bechtel PSQR's were aware af the staking requiranents ani would inspect all risirg,stem abator aperted valves for proper string.
'iherefore, we feel that we have adequately addressed generic problem which wiLL be finaUzed with the c1osecut af NCR 3424 and the Condi-tion Re~rt, P-960.
