.and Drawings, "Activities affecting quality shall be prescribed by documented instructions...and shall be accomplished in accordance with these instructions..."
The accepted Quality Assurance
- Program, TVA-TR75-1A, Revision 3, states in
- part, "Measuring and test equipment utilized in or related to operation of TVA's nuclear plants shall be controlled in accordance with written procedures or instructions."
Paragraph 3.4.1 of the Browns Ferry Operational Quality Assurance Manual (Part III, Section 3.1) states, "Measuring and Test Equipment whose cali-bration due date has passed...shall be clearly tagged to indicate its quality
- status, removed from service, and segregated from acceptable equipment."
Contrary to the above, the activities affecting quality were not accomplished in accordance with the instructions in that the calibration equipment, current calibrator C-4, was not tagged out, nor segregated from acceptable equipment on 10/24/79.
- Further, the equipment had been used 11 times to calibrate safety-related instruments in unit 3 since the last calibration due date of 8/6/79.
~Res ense Current calibrator C-4 was listed on the schedule for recalibration as stated on August 6, 1979.
It was erroneously overlooked when the shipping ticket was being compiled for shipment to the Chattanooga Central Laboratory.
Corrective Ste s Taken and 'Results Achieved Current calibrator C-4 was immediately removed from service,
- tagged, and placed in segregated storage.
All calibrations involving the use of this instrument were reverified.
This instrument was sent to the Central Laboratory and found to be within tolerance.
Corrective Ste s Taken to Avoid Further Noncom liance A computer program listing all items of measuring and test equipment utilized by the instrument maintenance section has been developed.
This listed is compared to the listing issued by the quality assurance staff.
First and second person verification is provided by the engineering aide and the plant calibration facility supervisor.
Instrument Maintenance Section Instruction
Letter 10 has been revised to require that the calibration due date for each piece of measuring and test equipment be checked before use.
