JAN 1 7 188 APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License Nos.

DPR-33 DPR-52 DPR-68 Based on the results of the NRC inspection conducted on December 4-8 and 18-20, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below.

These items have been categorized as described in our correspondence to you dated December 31, 1974.

Technical Specification 6.3.A.8 states that written procedures shall be prepared, approved and adhered to including Radiological Emergency Plan implementing procedures.

Contrary to the above, (1) on December 7,

1978, the inspector reviewed inventory records which indicated that the Emergency Kits on-site had not been inventoried during the third quarter of 1978 as required by Radiation Control Instruction RCI-4, and (2) on December 18, 1978, the inspector
observed, at Colonial Manor Hospital in Florence,
Alabama, two radiation survey instruments whose last calibration was performed on November 1,
1978, and November 13, 1978, respectively, thus exceeding the monthly calibration as required by Radiation Surveillance and Services Section inventory instructions.

This is an infraction and is a repeat item of noncompliance.
