NOV 2 7 197 APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License Nos.
DPR-33 DPR-52 DPR-68 Based on the results of the NRC inspection conducted on October 23-27, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in our correspondence to you dated December 31, 1974.
A.
Technical Specification 6.3.A required that detailed written procedures be prepared, approved and adhered to.
Technical Specification 6.3.A.7 identified Radiation Control Procedures as one of the categories of procedures to which the general statement in Technical Specification 6.3.A applied.
Paragraph I.C.3.b(3) of Radiological Control Instruction 3, "Respiratory Protection Program",
required that satisfactory respirator fit be verified by performance of an "amyle (sic) acetate or irritant smoke test".
Contrary to the above, licensee representatives failed to conduct an amyl acetate or irritant smoke test while fitting an NRC Inspector with a respirator on October 26, 1978.
This is an infraction.
B.
Technical Specification 6.3.D required that any individual or group of individuals permitted to enter high radiation areas be provided with a radiation monitoring device which continuously indicated the radiation dose rate in,the area.
Contrary to the above, the licensee was not providing a radiation monitoring device of the type required by Technical Specification 6.3.D, to each individua) or group of individuals entering a high radiation area.
This is an infraction.
