APPENDIX A NOTICE OF VIOIATION Tennessee Valley Authority License Nos.
DPR-33 DPR-52 DPR-68 Based on the results of the NRC inspection conducted on September 11-14, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in our correspondence to you dated December 31, 1974.
Technical Specifications 4.7.E specifies the surveillance requirements for the Control Room Emergency Ventilation, including the tests for HEPA filters and charcoal adsorbers.
Technical Specifications 4.7.F specifies the surveillance requirements for the Primary Containment Purge Systems, including the tests for HEPA filters and charcoal adsorbers.
The test interval specified by Technical Specifications 4.7.E and 4.7.F is at least once per operating cycle or 18 months, whichever occurs first.
Contrary to the above, the interval between the most recent tests of the Unit 2 Primary Containment Purge System was 24
- months, and tests of the Unit 3 Primary Containment Purge System and Control Room Emergency Ventilation were last performed in May, 1976.
This was determined by a review of plant surveillance records by an inspector on September 12, 1978.
This is an infraction.
