NOTICE OF VIOLATION American Electric Power Service

'orporation Docket No. 50-315 Docket No. 50-316 As a result of the inspection conducted on July 7-17 and September 15,

1987, and in accordance with the "General Policy and Procedures for NRC Enforcement Actions'," 10 CFR Part 2, Appendix C (1985), the following violations were identified:

1.

Technical Specification 3.7.9.3 requires a roving fire watch patrol in those areas where the low pressure carbon dioxide systems are isolated for personnel protection to permit entry for routine tours, maintenance, construction, or surveillance testing.

Contrary to the above,,on April 5, 1984, it was identified by a security officer that the carbon dioxide system was isolated with no firewatch as required by Technical Specification 3.7.9.3 for approximately two hours and forty minutes, The firewatch had failed to return the carbon dioxide isolation switch to the normal position, and a security officer had failed to verify that the switch was in the normal position.

In addition, on September 28, 1984, April 29,

1985, and October 30,
1985, carbon dioxide systems were isolated for personnel protection and no firewatches were provided as required by Technical Specification 3.7.9.3.

This is a Severity Level IV violation (Supplement I).

Technical Specification 3.7. 10.a requires a continuous firewatch or an hourly firewatch with verification of operable fire detectors on one side of an inoperable fire barrier.

Contrary to the above, on July 6, 1984, it was identified by the licensee that a firewatch was not maintained as required by Technical Specifications 3.7.10.

This event occurred in the northwest corner of the auxiliary building at elevation 573 where fire retardant material providing a fire barrier had been removed.

This condition lasted approximately four hours and 40 minutes.

In addition, on November 11,

1984, November 15,
1984, December 6 through December 17, 1984, January 5,

1985, August 9,

1985, and October 4, 1985, the required compensatory measures as required by Technical Specification 3.7.10.a were performed late or not at all.

This is a Severity Level IV violation (Supplement I).

The inspection showed that action had been taken to correct the identified violations and to'prevent recurrence.

Consequently, no reply to the violations is required and we have no further questions regarding this matter.
