MANAGEMENTACTlONS ST. LUCIE PLANT Fitness for Duty Reportable Event Qn August 21, 1995, a confirmed positive random test result was received on a supervisor for Cocaine. Telephone notification to the NRC was made at 7:35pm on August 21, 1995 (from the Turkey Point Plant). The individual was selected from the St. Lucie Plant random pool and the test was conducted at the Turkey Point Plant. A review was conducted, with negative results of the Subject's past work activities for a period prior to the test.

~A MANAGENIENTACTIONS TURKEY POINT PLANT 1

On September 26, 1995 a data input error occurred that resulted in 11 individuals obtaining access authorization but were not entered into the Fitness for Duty/Access Authorization data system to be made available for random chemical testing. All individuals were temporary workers and when all errors were identified the individuals had left the plant due to the completion of the outage.

The cause was human error by the individual responsible for data input. The immediate corrective action was to have each individual's supervisor provide a written behavioral observation review. There were no behavioral observation concerns identified. Additionally, the data input process was modified to require the first access authorization entry to be made in the Fitness for Duty/Access Authorization system prior to activating the individual's authorized site access.
