After Action Report/Improvement Plan River Bend Station EXECUTIVE

SUMMARY

On October 24, 2017, an out of sequence Reception Center drill was conducted for the Waterford 3 Steam Electric Station (W3) and the River Bend Station (RBS). Also, on October 25, 2017, an out of sequence Medical Services drill was conducted for RBS. Personnel from the U.S.

Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA)

Region VI, evaluated all activities. The purpose of these drills was to assess the level of preparedness of local responders to react to simulated radiological emergencies at the RBS and W3 on October 24, 2017 and at RBS on October 25, 2017. The previous reception center drill at this location was conducted on June 11, 2008, and the previous medical drill at this location was conducted on October 28, 2015.

Personnel from the State of Louisiana, River Bend Station, Baton Rouge River Center, American Red Cross, East Baton Rouge Parish, West Feliciana Parish Hospital, and West Feliciana Parish Emergency Medical Service participated in the drills. Cooperation and teamwork of all the participants was evident during the drills and DHS/FEMA wishes to acknowledge these efforts.

This report contains the final evaluation of the out of sequence drills. The participants adequately demonstrated knowledge of their emergency response plans and procedures. There were no Level 1 or Level 2 Findings identified during the drills. One Plan Issue was identified during the drills.

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region VI Office evaluated the drills on October 24-25, 2017 to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans and Procedures to protect the public health and safety during radiological emergencies involving River Bend Station (RBS) and Waterford 3 Steam Electric Station (W3). The purpose of this report is to represent the results and findings on the performance of the off site response organizations during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Exercise objectives and identified Capabilities/REP Criteria selected to be exercised are discussed in the Exercise Plan (EXPLAN), Appendix D.

2.3 Scenario Summary The drill scenarios were developed to evaluate the response of drill participants to an incident at the River Bend Station (RBS) and Waterford 3 Steam Electric Station (W3) on October 24, 2017 requiring the monitoring, decontamination, reception and care of evacuees, and to an incident at the RBS on October 25, 2017 requiring the transportation, treatment, and decontamination of a radiologically contaminated injured individual. The drill scenarios provided for the evaluation of the ability of the Baton Rouge River Center to monitor, decontaminate, and provide reception and care for evacuees for W3 and RBS, and provided for the evaluation of the ability of the West Feliciana Parish Hospital and West Feliciana Parish Emergency Medical Service to transport and treat a contaminated injured individual for RBS.

After Action Report/Improvement Plan River Bend Station SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the October 24 and 25, 2017 drills to test the offsite emergency response capabilities of state and local governments in the IO-mile Emergency Planning Zone surrounding the River Bend Station.

Each jurisdiction and functional entity was evaluated on the basis of its demonstration of criteria delineated in the exercise evaluation areas as outlined in the January 2016, Radiological Emergency Preparedness Manual. Detailed information on the exercise evaluation area criteria and the extent of play agreement used in this drill are found in Appendix D of this report.

3.2 Summary Results of Exercise Evaluation The matrix presented in Table 3.1 on the following page, represents the status of all exercise evaluation area criteria that were scheduled for demonstration during this exercise by all participating jurisdictions and functional entities. Exercise criteria are listed by number and the demonstration status is indicated by the use of the following letters:

After Action Report/Improvement Plan River Bend Station ISSUE NO.: 53-17-6al-P-Ol CRITERION: The reception center facility has appropriate space, adequate resources, and trained personnel to provide monitoring, decontamination, and registration of evacuees.

CONDITION: The Personnel Decontamination Station was initially set up in an incorrect location. The Vehicle Monitoring team when asked how they would direct the drivers and occupants of the contaminated vehicles, were uncertain what instructions they would provide the drivers and occupants. Additionally, the Vehicle Monitoring team was not certain how the vehicle occupants would return to the reception center for personnel monitoring (i.e.,

escorted, etc.). Nor did they have instructions to provide the drivers' and occupants of the vehicles if they were directed to Memorial Stadium. Procedures do not direct the Capital Area Transit Service to provide a shuttle service for the Memorial Stadium.

POSSIBLE CAUSE: The current procedure does not provide adequate detailed instructions for station set up, drivers of contaminated vehicles, or use of the personnel monitoring form.

After Action Report/Improvement Plan River Bend Station SECTION 4: CONCLUSION Based on the results of the drills, the off site radiological emergency response plans and preparedness for the State of Louisiana and the affected local jurisdictions are deemed adequate to provide reasonable assurance that appropriate measures can be taken to protect the health and safety of the public in the event of a radiological emergency. Therefore, 44 CFR Part 350 approval of the offsite radiological emergency response plans and preparedness for the State of Louisiana site-specific to River Bend Station and Waterford 3 Steam Electric Station will remain in effect.

Rev2 BATON ROUGE RIVER CENTER OUT OF SEQUENCE EVALUATION RECEPTION CENTER OCTOBER 24, 2017 INTRODUCTION In the interest of assuring the health and safety of the general public near the River Bend Station, in the event of a radiological emergency, drills periodically conducts drills jointly with the Federal, State and local agencies.

This manual contains the scenario of activities and supporting data describing the East Baton River Center Offsite Reception Center.

This scenario was developed by postulating events that will require response by offsite organizations.

During this drill, the support organizations will respond to the simulated events and conditions that have been selected to provide the level of activity necessary to meet the scenario objectives.

Baton Rouge River Center, American Red Cross, East Baton Rouge Parish Fire Department, and East Baton Rouge Mayor's Office of Homeland Security Emergency Preparedness (MOHSEP), and Louisiana Department of Social Services will participate in this drill.

Drill participants will have no prior knowledge of the sequence of events. The drill scenario will allow those individuals and agencies assigned to respond during a radiological emergency to demonstrate their performance according to current emergency preparedness plans and procedures.

The scenario is a mechanism by which selected drill controllers will initiate and evaluate the activities of the drill participants. The individual drills will be initiated as shown below and each is expected to last approximately three hours.

Participating organizations are shown in Section IV, Guidelines, of this manual.

Please note that this drill will also serve as a demonstration for both River Bend Station, as well as, Waterford 3 Nuclear Power Plant, since it is a shared reception center between both plants.

EVALUATION AREA 6: SUPPORT OPERATIONS/FACILITIES Sub-element 6.a - Monitoring, Decontamination, and Registration of Evacuees Criterion 6.a.1: The reception center facility has appropriate space, adequate resources, and trained personnel to provide monitoring, decontamination, and registration of evacuees. Participants will be pre-staged at or near the reception center.

b.

The capability to monitor 20% of Protective Action Sections 1-14 of the 10-mile EPZ over a 12-hour period will be demonstrated.

The population of these sections is 9,480. Twenty percent of this population is 1,896. To meet 20% requires a monitoring capability of 3 evacuees per minute.

c.

Per the REP Manual, a minimum of six evacuees will be monitored. Six people will simulate evacuees for the demonstration. At least one portal monitor will be set up and the six evacuees processed. One of the six will be simulated contaminated. One monitoring station for contaminated evacuees will be set up and the contaminated individual will be processed through decontamination. Handling of potentially contaminated personal belongings will be demonstrated by discussion.

d.

One vehicle will be monitored for contamination. The vehicle will not proceed to the segregated parking area after monitoring. The segregated parking area will not be set up to receive vehicles, but will be available for viewing and discussion.

Upon entry to the site protected area, security will provide the "ambulance kit" to EMS personnel. Contents of the kit will include a dosimeter of legal record (DLR) and a 0-200 mR pocket dosimeter for each individual. The actual kit contents will not be issued as part of the drill. A drill controller will provide drill dosimetry to the medics after kit contents have been shown to the evaluator.

EVALUATION AREA 3: PROTECTIVE ACTION IMPLEMENTATION Sub-element 3.a - Implementation of Emergency Worker Exposure Control Criterion 3.a.1: The OROs issue appropriate dosimetry, Kl, and procedures, and manage radiological exposure to emergency workers in accordance with the plans/procedures.

Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart. OROs maintain appropriate record-keeping of the administration of Kl to emergency workers. (NUREG-0654/FEMA-REP-1, J.10.e; K.3.a, b; K.4)

Crew members will receive a turnover of patient information from site first aid and radiation protection personnel. Two radiation protection personnel will accompany the patient to the hospital.

At the hospital, removal of the patient's clothing will be simulated. Decontamination will be demonstrated to the evaluators using methods contained in the hospital procedure. These methods could include soap and water, wipes, or tape. Intrusive bioassay samples will be simulated.

• No actual surgical procedures, X-ray, drawing of blood samples, etc. will be conducted.

A chemistry technician taking a water sample in the CST slips and falls on his way down the ladder.

During the fall, his hand hits a sharp object and is injured and contaminated with spilled water.

He is able to crawl out of the radiologically controlled area (RCA) where he is found by a co-worker, conscious, and bleeding slightly from a wound on his left hand and complaining of back pain. The co-worker will contact the control room for assistance.

Expected actions:

When contacted, the operations shift manager will implement the steps of ADM-0060, First Responder Emergencies. The First Responders will be contacted and radiation protection (RP) support requested.

The first aid team responding to the call will perform an assessment of the patient's condition and treat any life-threatening injuries. Based on the individual's condition and vital signs, the responding First Responders will make the decision to transport without decontamination. The control room will be contacted to arrange for ambulance and hospital support based on ADM-0060.

RIVER BEND STATION 2017 MEDICAL EMERGENCY RESPONSE EVALUATED DRILL EVALUATION AREAS AND EXTENT-OF-PLAY (EOP) AGREEMENT & ONSITE OBJECTIVES Radiation protection will monitor radiological exposures for both the responders and the patient. Contamination controls which do not impact care of the patient will be employed. Consideration should be given to the path the patient took from the original accident scene.

While awaiting ambulance arrival, the first aid team will perform the necessary actions to prepare the patient for transport. Assistance will be requested for patient transfer and the Primary Access Point (PAP) informed to prepare for ambulance arrival. RP will assist in planning the patient move to the ambulance pick up point.

The West Feliciana Parish EMS unit will be dispatched to the plant. The initial advisement to the ambulance will be that there is an injured individual located at the plant who is radiologically contaminated. Upon arrival of the ambulance, a turnover of patient medical and radiological conditions will be provided to the ambulance crew. RP will arrange for two technicians to provide radiological coverage for the patient and medical personnel while in route and at the hospital. As medical treatment and patienttransport evolve, contamination control measures will continue. Upon their departure from the plant, the ambulance personnel will communicate to West Feliciana Parish Hospital the medical and radiological information as well as an ETA (estimated time of arrival).

Upon notification of the situation, the hospital will prepare the medical team (anti-contamination clothing and dosimetry) as well as the REA (Radiation Emergency Area). When the ambulance arrives at the hospital, Emergency Department staff should meet the patient outside for the purpose of immediately determining the acuity of the case. The patient will be transferred into the REA. The ambulance personnel will be detained temporarily while a plant radiation protection technician surveys the ambulance personnel, equipment, and vehicle.

The patient will be evaluated as appropriate for their condition. A plant radiation protection technician will complete a thorough radiological survey of the patient and the findings should be verbalized and documented. The medical team will proceed with sample taking and decontamination. Radiological samples should be taken from contaminated areas of intact skin and wounds. Facial orifices should be sampled as well.

Decontamination efforts will be carried out until the radiological surveys indicate that background levels are obtained. Particular attention to the decontamination of the wound should be observed.

Following the successful decontamination of the patient, the patient exit process will commence. The patient should receive a full body survey prior to exiting the REA.

At this point the staff will exit the REA one at a time and follow the posted Staff Exit chart.
