This study was initiated by AEOD after the events were identified to Congress as Abnormal Occurrences in 1982.

This report documents AEOD's efforts, findings, and conclusions on the subject and reflects the results of the peer review process.

Recommendations from AEOD concerning this study have been previously forwarded to NRR for appropriate action X would be pleased to provide any clarification or further information that you may desire.
