SPECIAL REPORT February 8, 1988 FIRE PROTECTION SYSTEM IMPAIRMENTS REQUIRING NRC NOTIFICATION
~Psr ese:
Technical Specification (TS) 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration is not returned to functional status within 7 days.
TS 3.14 also states that certain other conditions require NRC notification.
In accordance with the above requirements, this special report is being generated in order to notify the NRC.
This report is being updated on a
30 day basis.
Event:
Turkey Point has been and is continuing to implement plant modifications.
Fire Protection System (FPS) impairments are, at times, incurred during the performance of these modifications.
Occasionally, certain FPS features fail due to random component failure.
Additionally, this report includes deficient penetrations and barriers which were identified during the 18 month periodic surveillance of penetrations..
Cause and Corrective Actions:
The penetrations and barriers identified during the periodic surveillance are considered to be impaired because they do not meet the design s minimum requirements The surveillance being performed presently, incorporates inspection criteria which are more stringent than the previous surveillance.
As a result, many penetrations and barriers are being identifi'ed as impaired, whereas previously they would have met the then existing criteria.
Additionally, certain impairments have been identified which are due to elastomer shrinkage.
The primary cause of the impairments which were not identified as a result of the periodic surveillance is the number of modifications being performed.
Many of these modifications require the impairment of certain FPS features for their completion.
The secondary cause for the impairments is FPS equipment malfunction.
Because Turkey Point incurs a variable number of impairments, the plant is reporting the impairments in a monthly report, rather than on a case-by-case basis.
The impairments reported this month may be either open or closed at the present time, however they were open in excess of the Technical Specification permitted time limit.
This report includes impairments not reported previously which were:
1) opened prior to January and 2) stayed open for longer than the time permitted by the Technical Specifications, and 3) are either still open or were closed prior to January.
The closed impairments have been reported at the time opened, unless noted otherwise.
The plant continues to implement the compensatory measures as applicable.
