APPENDIX A HOTICE OF VIOLATION Florida Power and Light Turkey Point 3 and 4

License Hos. DPR-31 and DPR-41 Based on the HRC inspection June 11-15, 1979, certain of your activities were apparently not conducted in full compliance with HRC requirements as indicated below.

These items have been categorized as described in correspondence to you dated December 31, 1974.

A.

As required by Section 6.8.1 of the "Technical Specifications" written procedures and administrative policies that meet or exceed the re-quirements and recommendations of Section 5.1 and 5.3 of ANSI H18.7-1972 and Appendix "A" of USNRC Regulatory Guide 1.33 shall be implemented.

Contrary to the above, the following three examples represent failure to implement existing procedures:

1)

Ho safety evaluation was performed as required by AP 190.15, "Plant Changes and Modifications" for the plastic sleeve placed over the B Monitor Tank overflow line resulting in the siphon effect

that led to the unplanned release of contaminated water on June 11, 1979.

2)

Recirculation of Monitor Tanks without use of paragraph 8.4 of OP 5163.1, "WDS-Waste Condensate Tank, Polishing Demineralizer and Monitor Tank Operation, "which addresses this evolution.

3)

Failure to log the unplanned liquid release on June 11, 1979, and the fluid transfer evolution taking place prior to the release in the Nuclear Operator's log book, as required by paragraph 8.4.2.3 of AP 103.2, "Duties and Responsibilities of Operators on Shift and Maintenance of Operating Logs and Records."

This is an infraction B.

As required by Section 6.8.1 of the "Technical Specifications" written procedures and administrative policies that meet or exceed the require-ments and recommendations of Section 5.1 and 5.3 of ANSI H18.7-1972 and Appendix "A" of USHRC Regulatory Guide 1.33 shall be established.
