NRC1NSPECTION REPORTS:50-315/97003(DRS);

50%16/97003(DRS) AND

NOTICE OF VIOLATION

Dear Mr. Fitzpatrick;t~.

On March 14, 1997, the NRC completed an inspection at your D. C. Cook Units

1 and 2

reactor facilities. On March 31, 1997, a telephone exit was conducted to inform you that

an unresolved item discussed

in the exit conducted

on March 14, 1997, would become a

potential violation. The enclosed report presents the. results of the inspection.

Based on the results of this inspection, the NRC has determined that two violations of NRC

requirements occurred..The violations are cited in the enclosed Notice of Violation (Notice)

and the circumstances

surrounding them are described in detail in the subject inspection

report.

The first example involves the failure to correctly. specify procedures which are to

be used "in hand".

The NRC continues to be concerned that procedures were not being

rigorously implemented.

The second violation involves the failure to conduct a comprehensive

requalification annual

operating test for licensed operators.

We are concerned that licensed operators were

returned to licensed activities without meeting the requirements of 10 CFR Part 55.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response.

The NRC will use your response,

in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
