NRC SPECIAL RADIATIONPROTECTION INSPECTION REPORTS
50-315/98013(DRS); 50-316/98013(DRS) AND NOTICE OF VIOLATION
Dear Mr. Sampson:
On June 2, 1998, the NRC completed a special inspection, at your D. C. Cook, Units 1 and 2
reactor facilities. The enclosed report presents the results of that inspection.
The inspection was an examination of activities conducted under your license as they relate to
radiation safety and to compliance with the Commission's rules and regulations and with the
conditions of your license.
The purpose of the inspection was to follow up on an event where a
worker entered containment with an open wound without prior notification of the presence of the
wound to radiation protection personnel.
The enclosed report documents the results of the
inspection.
One violation of NRC requirements was identified, as specified in the enclosed Notice of
Violation (Notice). The violation concerned the failure to notify radiation protection of an open
wound prior to entry into a radiologically controlled area.
This violation is of concern because
there were several opportunities to notify radiation protection of the wound. Neither the worker,
his foreman, or the safety supervisor notified radiation protection of the presence of the wound.
The NRC has concluded that information regarding the reason for the violation, the corrective
actions taken and planned to correct the violation and prevent recurrence,
is already adequately
addressed
in the subject inspection report. Therefore, you are not required to respond to this
violation unless the description of the problem as described in the inspection report does not
accurately reflect your corrective actions or your position.
In that case, or ifyou choose to
provide additional information, you should follow the instructions specified in the enclosed
Notice.
In accordance with 10 CFR 2.790 of the NRC'S "Rules of Practice," a copy of this letter, the
enclosures, and your response to this letter, ifyou choose to provide one, willbe placed in the
NRC Public Document Room (PDR).
