D. C. COOK INSPECTION REPORT 50-315/98007(DRP); 50-316/98007(DRP)
Dear Mr. Sampson:
On April27, 1998, the NRC conducted a re-exit on the findings from an inspection completed on
March 13, 1998, at your D. C. Cook Units 1 and 2 reactor facilities. The enclosed report presents
the results of that inspection.
The re-exit was conducted to inform you of the deliberations
regarding the potential enforcement issues discussed
in this report.
During the period covered
by this inspection report, the inspectors observed that the plant was safely maintained in a
shutdown condition.
Radiological work practices were adequate
and maintenance work
procedures were properly followed.
This inspection was conducted to assess
two systems important to containment pressure
suppression.
The systems selected were the hydrogen recombiner system and the distributed
ignition system.
For the hydrogen recombiner system, the inspectors identified several examples
of failures to comply with Technical Specification (TS) surveillance requirements along with
several examples of inadequate procedures.
In the case of the distributed ignition system, the
inspectors identified that although the system did not have TS surveillance requirements,
commitments were identified to perform tests.
The inspectors identified examples of deviations
from your commitments to perform tests.
The details pertaining to each apparent violation are
described in the enclosed report.
Based on the results of this inspection, six apparent violations of NRC requirements were
identified. No Notice of Violation is presently being issued for these inspection findings.
In
addition, the number and characterization of apparent violations described in the enclosed
inspection report may change as a result of further NRC review.
You willbe advised by separate correspondence
of the results of our deliberations on this matter.
No response regarding these apparent violations is required at this time.
