NRC INSPECTION REPORT 50-315/97023(DRS); 50-316/97023(DRS)

Dear Mr. Fitzpatrick:

On November 21, 1997, the NRC completed an inspection at your Donald C. COOK Nuclear

Generating Plant. The enclosed report presents the results of that inspection.

The area examined during this inspection was the effectiveness of your controls in identifying,

resolving and preventing

issues that degrade the quality of plant operations.

The purpose of

the inspection was to determine whether activities authorized by the license were conducted

safely and in accordance with NRC requirements.

In response to continuing issues being surfaced in your internal audits and self-assessments

and additional concerns being raised by the NRC regarding your effectiveness

in identifying,

resolving and preventing problems, you initiated a number of improvement initiatives.'he

inspectors noted that progress had been made in initiating steps to improve the corrective

action program and strengthen self-assessment

activities. Since implementation of the revised

corrective action program in May 1997, you continue to be effective in identifying plant

problems, however, the resolution and final closure of these problems continue to be slow and

not effective. Of particular concern is the untimely resolution of issues identified by your staff.

For example, during this inspection, we noted that your self-assessments

found

that issues identified several years earlier during internal audits had not yet been corrected.

A

second example involves the slow follow up on the Corrective Action Continuous Improvement

Group recommendations

to improve the corrective action process.

These items are discussed

in the enclosed inspection report. We understand that you are aware of these issues and are

working to further improve the performance of your corrective action process.

You are

encouraged to continue your progress in this area and are reminded that your attention to

timely and thorough corrective actions must be maintained.

With respect to self-assessments

and audits, the process observed appears to be sound.

In most cases,

problem identification

appeared to be satisfactory.

During the latter part of this inspection period, the inspectors became aware that in two recent

self-initiated audits, you identified a number of issues involving inadequate training and

qualifications of engineering personnel.

We request that within 30 days of this letter, you

provide this office with the actions you plan to take to address these training and qualification

deficiencies.
