REPORT NO. 50-315/97009(DRP);

50-316/97009(DRP)

Dear Mr. Fitzpatrick:

On May 23, 1997, the NRC completed an OSTI at your D. C. Cook

1 and 2 reactor

facilities, and discussed

the results of the inspection with you at an exit

meeting.

The enclosed report presents the results of that inspection.

The OSTI was conducted to determine the scope.and

magnitude of recent problems with

procedure adherence

and adequacy, the conduct of operations, and with material

condition.

No significant issues were identified during this inspection relative to

procedures.

We willcontinue to assess

procedure use and adherence

as you implement

recent changes to your governing procedure.

In addition, your staff made three procedure-

related commitments to us to address discrepancies

involving consistency of component

nomenclature, flow diagrams, and sealed valves.

These commitments are discussed

in

Section 03.1.

If our characterization of the commitments differs from your understanding,

please inform us by letter.

Overall, control room and in-plant operations department activities were conducted in a

professional manner., In general, we found good control room decorum, operators

attentive to their job related duties, and use of three way communications.

However, a

mispositioned valve went undetected

by Unit 1 control room operators for a period of 12

hours.

Followup to this mispositioned valve revealed that the operations department did

not have documented

requirements regarding how frequently operators should walk down

control room panels.

While this event appears to be isolated, your corrective action for

this problem needs to address whether or not routine panel walkdowns by plant operators

are conducted at,a frequency that willidentify rnispositioned valves in a timely manner.

The material condition of major components

in the auxiliary building was good.

However,

water and oil leaks persist.

For example, leaks continue to exist on the Unit 1 main

feedwater pumps.

These leaks remain even though Unit 1 recently carne out of a refueling

outage in which repairs were made to the pumps.

Based on the resuits of this inspection, two violations of NRC requirements were

identified. The first violation involves two examples of proceduie adherence

issues.

The

first example pertains to temporary modifications that remained in-place past the,

procedure-allowed

extension date.

While only one instance is cited in the inspection

report, the inspectors identified six other temporary modifications on nonsafety-related

systems that were similarly past the extension dates.

The second example involves the

inability to follow the appropriate procedure for deenergizing the 2AB emergency diesel

generator inverter.

Because your staff has provided the NRC with information regarding

the reasons for the violation and the corrective actions taken and planned to correct both

examples of the violation and prevent recurrence, as described in the enclosed Inspection

Report, you,are not required to respond to this letter unless the description therein does

not accurately reflect your corrective actions. or your position.

In that case, or if you

,choose to provide additional information, you should-follow the instructions specified in

the enclosed Notice.

The second violation was identified during our review of four recently installed

modifications.

In three of the four modification packages,

our inspectors identified

discrepancies,

suggesting that additional effort is necessary to ensure that complete and

accurate information is included in modification packages.

You are required to respond to

this violation and should follow the instructions specified in the enclosed Notice when

preparing your response.

The NRC will use your response,

in part, to determine whether

further enforcement action is necessary to ensure compliance with regulatory

requirements.
