Attachment 1 is our notification to the Michigan Department of Environmental Quality (MDEQ), dated July 22, 1997, regarding unit 1 (outfall 001) analyzer calibration drift.

Attachment 2 is our notification to the MDEQ, dated July 25, 1997, regarding exceedence of total residual oxidant in the daily continuous chlorination of the unit 1 circulating water system.

Dear Mr. Morley:

This notification is made pursuant to Cook Nuclear Plant's NPDES Permit MI0005827, Part II, A.5.

On July 14, 1997, the Unit 1 (outfall 001) analyzer was calibrated as part of the twice-weekly calibration schedule.

When the evaluation of calibration data was completed on July 18, 1997, it was determined that the analyzer had drifted in a non-conservative direction.

It is estimated that the readings logged on July 13, 1997, were actually 20 times higher than indicated.

Therefore, the three readings of 6, 15, and 20 pg/I were calculated to be 120, 300, and 400 pg/I, respectively.

The daily maximum intermittent value is estimated to be 273 Ijg/I. The Total Residual Oxidant daily maximum intermittent limitfor Outfall 001 is 200 pg/I, with a single sample limitof 300 yg/I.

The exceedence occurred during intermittent chlorination on July 13, 1997, from 1050 to 1130 EDT. There was no exceedence of the continuous chlorination limit.

The analyzer was recalibrated and returned to service on July 14, 1997.

The cause of the analyzer drift is under investigation.

Dear Mr. Morley:

This notification is made pursuant to Cook Nuclear Plant's NPDES Permit MI0005827, Part II, A.5.

On July 21,

1997, during the daily continuous chlorination of the Unit One Circulating Water System, the Outfall 001 daily maximum was measured at 0.013 ppm Totyl Residual Oxidant (TRO). This value exceeds the permitlimit of 0.006 p

d '~ii ft kM Permit MI0005827. Chlorination was terminated upon discovery of the exceedence.

The cause of the exceedence was a failed back pressure regulator.

The regulator adjusting device on the sodium hypochlorite feed pump skid loosened due to mechanical vibration.

As a corrective measure, all back pressure regulators associated with the chlorination system were modiTied to protect them from similar failures caused by mechanical vibration.
