NRC INSPECTION REPORT 50-315/316-97004(DRP),

NOTICE OF

VIOLATION,AND NOTICE OF DEVIATION

Dear Mr. Fitzpatrick:

On March 29,,1997, the NRC completed an inspection at your D. C. Cook

1 and 2 reactor

facilities. The enclosed report presents the results of that inspection.

During the 6-week period covered by this inspection report, the inspectors observed that

the plant was operated in a safe manner, radiological work practices were properly

followed, and engineering evaluations were promptly performed.

Based on the results of this inspection, four violations of NRC requirements and a deviation

from a commitment were identified. The violations are cited in the enclosed Notice of

Violation (Notice), and the circumstances

surrounding them are described in detail in the

subject inspection report.

The first violation identified four examples of failure to follow

procedures including an operator error that resulted in an inadvertent engineered safety

feature actuation.

The second violation identified two examples of inadequate corrective

action to prevent recurrence of significant conditions adverse to quality. The third

violation involved a failure to make a timely 10 CFR 50.72 report.

Violation four involved

the failure to properly evaluate a temporary modification. The identified deviation related

to the capability.of equipment to properly record power range channel data.

Procedural quality, employee and contractor adherence to procedures,

quality of corrective

actions, and the need to comply with reporting requirements have been the subjects of our

letters to you in previous inspection reports.

Most recently in our letter to you dated

March 27, 1997, transmitting inspection report No. 50-315/316/97002(DRP),

we

expressed

our concern with the frequency of procedural adherence

and inadequate

procedure problems.

In this enclosed report we again identified that these problems still

continue.

The number and type of violations suggest that continued improvement needs

to be made in the areas of procedural quality and adherence,

corrective actions, and the

ability to make timely reports.
