Dear Hr. Fitzpatrick:
Enclosed for your review is the Systematic Assessment of Licensee Performance (SALP) 13 Report for the Donald C.
Cook Nuclear Plant, covering the period October 30, 1994 through.Hay 25, 1996.
A public meeting to discuss this report with you and your staff has been scheduled at the Donald C.
Cook Visitors Center, on July 25, 1996, at 1 p.m.
(EDT).
During this meeting you are encouraged to candidly discuss any comments you may have regarding our report.
In accordance with NRC policy, I have reviewed the,SALP Board assessment and concur with its ratings.
The area of plant support was rated Category 1.
The areas of plant operations, maintenance, and engineering were each rated Category 2.
The details of our assessment are provided in the enclosed report.
It is my view that conduct of activities at the D. C.
Cook nuclear plant, while good overall, declined in operations and maintenance when compared to the last SALP period.
This was primarily due to operational and maintenance errors leading to several plant trips, transients, and operability concerns.
Overall plant performance was good and generally reflected a conservative operating philosophy and good safety focus.
Strengths were observed in the experience and professionalism of the operators, continued low collective station personnel
- exposure, excellent emergency drill performance, and effective maintenance of security systems.
- However, performance in plant operations and maintenance
- declined, driven by issues of poor communications between departments, personnel
- errors, procedural adherence
- problems, lack of timely identification and resolution of issues and ineffective work planning and scheduling.
- Further, balance of plant equipment problems resulted in plant trips, transients and forced shutdowns.
While overall engineering activities remained good, in some instances, engineering staff exhibited inadequate awareness, understanding, and use of the plant design and licensing bases.
Engineering was also weak in the timely identification and resolution of'issues, but once clearly focused on a problem, engineering efforts to resolve issues and support operation of the plant were good.

E. Fitzpatrick July 8, 1996 Early this year, during a management meeting with the NRC staff, you outlined the initiatives you had implemented to improve plant performance.
The recent Integrated Performance Assessment Program team inspection and subsequent inspections noted some improvement in operational performance as a result of increased management oversight.
We view these as positive plans which should arrest the decline in performance and encourage you to continue with these efforts.
In accordance with Section 2.790 of the NRC's "Rules of Practice,"
Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the SALP report will be placed in the NRC's Public Document Room.
Should you have any questions concerning the SALP Report, we would be pleased to discuss them with you.
While no written response is required, if you wish, you may provide written comments within 30 days of the SALP meeting.
