FOLLOW-UP RESPONSE TO NOTICE OF VIOLATION This letter is a follow-up to our response to the Notice of Violation contained in Inspection Report
¹95009, forwarded to Indiana Michigan Power in a letter from W.
L.
Axelson dated September 12, 1995.
The original notice cited three examples of failure to follow procedures, of which one involved operators not reducing power during a loss of main turbine condenser vacuum transient.
In the response provided in AEP:NRC:1224B, dated October ll, 1995, we stated that we considered the actions to be taken for the loss of main condenser vacuum to be an open issue.
We committed to continue our evaluation of this event to determine the most appropriate actions for loss of main condenser vacuum.
This evaluation has been completed.
Our evaluation concluded that the decision to not decrease power in response to the loss of vacuum was not in line with our conservative operating philosophy, or with that of the industry in general.
Consequently, we have provided procedural guidance and shift training to direct the operating crew to reduce power immediately in response to any confirmed loss of vacuum condition.
This results in actions more in keeping with our conservative operating philosophy and minimizes the magnitude of the transient should the need to trip the plant arise.
We have also determined the need for a 'apid shutdown'rocedure to guide operating crews for conditions in which a rapid power reduction would be prudent.
To meet this need, a rapid shutdown procedure has been developed for the Donald C. Cook Nuclear Plant, and training on this procedure has commenced.
