REPORT 50-315/316-95013 (DRP)
AND NOTICE OF VIOLATION-
Dear Mr. Fitzpatrick:
This refers to the routine safety inspection
conducted
by Messrs.
B. Bartlett,
D. Hartland,
C. Orsini,
R. Paul,
B. Fuller,
and
N. Shah of this office from
December
5,
1995,
through January
16,
1996.
The inspection
included
a review
of activities at your Donald C.
Cook Nuclear Plant,
Units
1 and 2.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed report.
Based
on the results of this inspection,
the
NRC has determined that two
violations of NRC requirements
occurred.
The violations are cited in the
enclosed
Notice of Violation (Notice)
and the circumstances'surrounding
them
are described
in detail in the subject inspection report.
The violations are
of concern
because
in one case
a Technical Specification requirement for. the
diesel
generators
was not properly implemented,
and in the other case there
was
a failure to test safety-related
equipment
under conditions
assumed
in
design basis
documents.
We are concerned
that during this inspection there were several
issues
identified which pertained to implementation of technical specification
requirements
in the area of surveillance testing.
There were also continuing
performance
issues
identified in the work control process,
two of which
pertained
to post maintenance
testing.
You.are required to respond to this letter and should follow',the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken -and any additional
actions
you plan to- prevent recurrence.
Your response
may reference
or
include previous docketed
correspondence, if the correspondence
