REPLY TO NOTICE OF VIOLATIONS This letter is in response to a letter from W. J. Kropp dated December 28, 1995, that forwarded a notice of violation to Indiana Michigan Power Company. The violation was identified during a routine safety inspection conducted by Messrs. Bartlett, Hartland, Orsini, Dunlop, Winter, and Belanger from October 12 through December 4, 1995. The violation is associated with debris inside the unit 1 containment building.

Our reply to the violation is provided in the attachment to this letter. The reply does not contain any personal privacy, proprietary, or safeguards information.

ATTACHMENT TO AEP'NRC:1224E Page 1 During an NRC inspection conducted October 12 through December 4, 1995, a violation of NRC requirements wa" identified. In accordance witn the "General Statement of Policy and Procedures for NRC Enforcement Actions," (60 FR 34381, June 30, 1995) the violation and the Donald C. Cook Nuclear Plant response are provided below:

procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, Rev. 2, February 1978.

Regulatory Guide 1.33, Quali.ty Assurance Program Requirements (Operation), Appendix A, states, in part, that startup and operation of the emergency core cooling system should be covered by written procedures.

Paragraph No. 13.0 of procedure No. OHP-4030.001.002, "Containment Inspection Tour," required that all trash and loose debris be removed from all areas of upper containment and ice condenser.

Contrary to the above, on October 16, 1995, the inspectors discovered trash and debris in Unit 1 upper containment and ice condenser following completion of the licensee's refueling outage close-out tour.

This is a Severity Level IV violation (Supplement I)."

ATTACHMENT TO AEP'NRC'1224E Page 2 control system. These action requests will ensure the containment inspection tour receives the appropriate level of planning as required within the work control system.

Additionally, a new Radiation Protection (RP) procedure will be written detailing containment cleanliness requirements.

This procedure will provide an appropriate level of detail to result in a successful containment cleanliness tour. The RP procedure is scheduled for issuance by April 30, 1996, prior to the completion of 'the next scheduled refueling outage in which it will be used. The next scheduled refueling outage is on Unit 2 and is scheduled to begin in March of this year.
