SPILL NOTIFICATION The attached notifications are forwarded for your information pursuant to Appendix B of the Donald C.

Cook Nuclear Plant Environmental Technical Specifications.

The subject of these notifications concern a spill of ethylene glycol monobutyl ether (high expansion foam from a fire suppression system) to the ground, and a spill of transformer oil to the catch basin and the ground.

Ethylene Glycol Monobutyl Ether Spill Notification NPDES Permit No. MI0005827

Dear Mr. Morley:

The following notification is being performed per NPDES Permit MI0005827, Part II.

Section A.6 '

Spill Notification.

The spill was previously reported via a telephone call to Mr. Greg Danneffel of the Michigan DNR Plainwell office at 4:00 p.m.

on August 16, 1995.

The spill occurred at approximately 3:30 p.m.

on August:

15, 1995 when a lightning strike caused a

spurious discharge of the fire suppression system at an on site warehouse.

.An estimated 36.3 gal. of High Expansion Foam (see attached MSDS) containing 104 lbs. of ethylene glycol monobutyl ether mixed with fire suppression water reached the ground.

No remediation of the surrounding soil was performed.

We are undertaking an investigation to determine actions to prevent future spill of this nature.

Dear Mr. Morley:

The following notification is being performed per NPDES Permit MI0005827, Part II.

Section A.6.,

Spill Notification.

The spill was previously reported via telephone calls to the Michigan DNR PEAS Operator 32 at 4:45 a.m.

on August 20, 1995 and Ms. Lorraine Thomas of the Environmental

Response

Division at 2:00 p.m.

on

'August 21, 1995.

The spill occurred at 00:49 a.m.

on August 20, 1995 when the Cook Plant, Unit 1 Main Transformer experienced a

phase to phase internal arc

over, which caused a

catastrophic failure;of the transformer.

Approximately 600 gallons of non-PCB mineral oil (Ultravolt 60) were

released, of which 50 gallons were spilled to the ground surrounding the concrete transformer catch basin.

Per

design, the bulk of the spill went via the bermed containment area to an oil/water separator where it is being recovered.

Approximately 10 cubic yards of oil contaminated soil was removed.

The soil was removed to a layer where there was no visible contamination or odor. Oil-Dry was used to clean up stains on the asphalt'.

and concrete, and the steel surfaces were wiped down with absorbents.

No oil was spilled to Lake Michigan, or reached the groundwaters of the State.
