(SALP)
12 Report for the Donald C.
Cook Nuclear Plant, covering the period
Hay 1,
1993 through October 29,
1994.
As discussed
between
Hr. A. Blind of
your staff and Hr. J.
Isom of this office,
a meeting to discuss this report
with you and your staff has
been
scheduled
at the Donald C.
Cook Visitors
Center,
on December
20,
1994, at 10:00 a.m.
(EDT).
During this meeting you are encouraged
to candidly discuss
any comments
you
may have regarding
our report.
While this meeting is considered
a
presentation
and discussion
forum between
Company
and
the
NRC, the meeting will be opened to any other interested
parties
as
observers.
In accordance
with Nuclear Regulatory
Commission
(NRC) policy, I have reviewed
the recommendations
resulting from the
SALP Board assessment
and concur with
their ratings.
Overall, your performance
was excellent
and generally reflected
a conservative
operating philosophy
and safety focus.
The fu~ctional areas of Operations,
Maintenance,
and Plant Support were assigned
Category
1 ratings.
Excellent
performance
in these
areas
was demonstrated
by few personnel
errors
and the
high reliability and availability of equipment.
Strong
management
involvement
in the latter part of the
SALP period significantly contributed to the
improved performance
in Operations
and Plant Support.
The Engineering
functional
area
was rated
a Category
2.
Engineering
performance
was generally
strong but there
were instances
where technical
evaluations of emergent
equipment
problems
were not sufficiently rigorous
and weaknesses
were noted in
the knowledge
and application of design information.
Your efforts in self identification of deficiencies
were considered
good.
A
commitment to self-assessment
within the line organization
was evident
by the
self assessments
in system engineering
and chemistry.
However, there were
some performance
lapses
in engineering
and operations
which point to the need
for the plant staff to have
a continuous self-critical attitude regarding the
assessment
of work.
The implementation of an effective problem
identification, evaluation,
and assessment
process will be important for
sustaining
improved performance
at D.

evaluations for potential
safety concerns.

two issues
pertained to
increasing
component cooling water temperature
to reduce reactor coolant
pump
seal
leakage
and loose reactor
head penetration
funnel guides.
We were unable
to determine if these
two instances
were isolated occurrences
or represent
a
pattern.
We encourage
you to determine if there
have
been other occurrences
that could point to underlying problems in addressing
emergent
issues
without
adequate
consideration for potential safety consequences.
