Dear Mr. Russell:

As requested by your staff in a telephone conference with American Electric Power Service Corporation personnel on October 24, 1994, we are providing our CRDM Reactor Vessel Head Penetration Assessment for your review and approval.

Attachment 1 contains Westinghouse Electric Corporation's "Assessment of Indications In Donald C. Cook Unit 2 Head Penetration 75," which addresses the as-found conditions and how we are bounded by the analysis of the generic WCAP-13565, "Alloy 600 Reactor Vessel Head Adaptor Tube Cracking Safety Evaluation."

The NRC's "Safety Evaluation for Potential Reactor Vessel Head Adaptor Tube Cracking," dated November 19, 1993, responded to and accepted WCAP-13565. As a result of the inspection and Westinghouse's assessment of the CRDM penetration number 75, we intend to perform another inspection during the next refueling outage. Attachment 2 contains Westinghouse Electric Corporation's report WCAP-14118, Revision 1, entitled "Structural Integrity Evaluation of Reactor Vessel Upper Head Penetrations to Support Continued Operation: D. C. Cook Unit 2." The report is considered proprietary to Westinghouse Electric Corporation. As such, an affidavit for withholding from public disclosure is provided in Attachment 3.

Mr. W. T. Russell 2- AEP:NRC'1218 The current schedule indicates that Unit 2 could be ready to enter mode 4 late Friday night, October 28, 1994. We do not intend to enter mode 4 until we receive notification that our report has been approved. Mode 4 was chosen as the constraint since this is where our technical specifications for pressure boundary leakage apply.
