Dear Mr. Russell:
As requested by your staff in a telephone conference with American Electric Power Service Corporation personnel on October 24,
- 1994, we are providing our CRDM Reactor Vessel Head Penetration Assessment for your review and approval.
Attachment 1
contains Westinghouse Electric Corporation's "Assessment of Indications In Donald C.
Cook Unit 2
Head Penetration 75," which addresses the as-found conditions and how we are bounded by the analysis of the generic WCAP-13565, "Alloy 600 Reactor Vessel Head Adaptor Tube Cracking Safety Evaluation."
The NRC's "Safety Evaluation for Potential Reactor Vessel Head Adaptor Tube Cracking," dated November 19,
- 1993, responded to and accepted WCAP-13565.
As a
result of the inspection and Westinghouse's assessment of the CRDM penetration number 75, we intend to perform another inspection during the next refueling outage.
Attachment 2 contains Westinghouse Electric Corporation's report WCAP-14118, Revision 1,
entitled "Structural Integrity Evaluation of Reactor Vessel Upper Head Penetrations to Support Continued Operation:
D. C. Cook Unit 2."
The report is considered proprietary to Westinghouse Electric Corporation.
As such, an affidavit for withholding from public disclosure is provided in.

Mr. W. T. Russell 2-AEP:NRC'1218 The current schedule indicates that Unit 2 could be ready to enter mode 4 late Friday night, October 28, 1994.
We do not intend to enter mode 4 until we receive notification that our report has been approved.
Mode 4 was chosen as the constraint since this is where our technical specifications for pressure boundary leakage apply.
Therefore, your expeditious review and approval is greatly appreciated.
