ROUTINE CHEMISTRY INSPECTION AT THE D. C.

COOK NUCLEAR PLANT

ON JULY 18

THROUGH 22,

1994

This refers to the routine safety inspection

conducted

by Dr. John

House

and

Hr. Steven Orth of this office on July 18-22,

1994.

The inspection

included

a

review of authorized activities at your D. C.

Cook Nuclear Plant facility.

At

the conclusion of the inspection,

the findings were discussed

with those

members of your staff identified in the enclosed report.

Areas examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted of a selective

examination of procedures

and representative

records,

interviews with personnel,

and observation of

activities in progress.

In addition,

our Hobile Laboratory

was at the site

during the inspection to perform independent

measurements

of radioactivity.

Based

on the results of this inspection

and your identification of repetitive

problems in the chemistry area,

we were concerned with the quality and depth

of your staff's evaluations

and corrective actions associated

with chemistry

program weaknesses.

The strengths

in identifying problems

have

been

overshadowed

by weaknesses

in problem evaluation

and resolution.

Root cause

evaluations

appeared

to be narrowly focused,

and corrective actions

were not

comprehensive. which resulted in repetitive. problems in your chemistry program.

We encourage

your aggressive self identification of problems

and acknowledge

the recent

steps

you have taken to strengthen

chemistry management,

however,

your evaluation

and resolution of problems

must continue to improve in order

to effect positive programmatic

changes.

Two non-cited violations were

identified.

In accordance

with 10 CFR 2.790 of the Commission's regulations,

a copy of

this letter and the enclosed

inspection report will be placed in the

NRC

Public Document

Room.
