This refers to the routine safety inspection
conducted
by Hessrs.
James
A.
Isom,
David J. Hartland, Christine A. Gainty, Joel
G.
Guzman,
and
Edward
R.
Schweibinz of this office from April 23,
1994,
through June 3,
1994.
The
inspection
included
a review of activities at your Donald
C.
Cook Nuclear
Plant, Units
1 and 2, facility.
At the conclusion of the inspection,
the
findings were discussed
with those
members of your staff identified in the
enclosed report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of a selective
examination of procedures
and representative
records,
observations,
and interviews with personnel.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation is of concern
because
you failed to perform
an engineering
evaluation of a motor-operated
valve
(HOV) that was in an over-
thrust condition.
We a'e also concerned
with the lack of progress
made
by
the engineering staff in validating assumptions
used in your
HOV program.
The
need to justify programmatic
assumptions
was identified as
an unresolved
item
during
a previous inspection
(50-315/93006-02(DRS);
50-316/93006-02(DRS))
and
will be
an important aspect to ensure timely GL 89-10 closure.
Additionally, during this inspection,
another activity appeared
to be in
violation of NRC requirements.
The violation was categorized
at Severity
Level
V and is not being cited because
the criteria specified in 10 CFR Part 2, Appendix C,Section VII.B(l) were satisfied.
