Dear Mr. Fitzpatrick:
This refers to the special
safety inspection
conducted
by Messrs.
James
A.
Isom and David J. Hartland of this office from April 4 through
22,
1994.
The inspection
focused
on the event involving the accumulation of silt in the
essential
service
water
(ESW)
supply lines to the Unit
1
and
2 "West" motor-
driven auxiliary feedwater
(AFW) pumps.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the
enclosed
report.
Areas examined during the inspection are identified in the report.
Within these
areas,
the inspection
consisted
oF
a selective
examination
oF procedures
and
representative
records,
observations,
and interviews with personnel.
Based
on the results of this inspection,
certain of your activities appeared
to
be
in violation of
NRC requirements,
as specified
in the
enclosed
Notice of
Violation (Notice).
The violation is of concern
because
the procedure
used to
flush the
ESW supply lines to the
AFW pumps
was
inadequate
to ensure
that
no
foreign material would accumulate
and possibly prevent sufficient ESW flow to the
pumps
upon demand.
Your response
to this violation should
include any additional actions to be taken to prevent other infrequently used
cooling loops from being subjected
to
a buildup of silt or other foreign matter
that could cause
a system
dependent
on
ESW to become
You are required
to respond
to this letter
and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your response,
you should
document
the specific actions
taken
and
any additional
actions
you
plan
to prevent
recurrence.
After reviewing your response
to this Notice,
including your proposed corrective actions
and the results of future inspections,
the
NRC will determine
whether further
NRC enforcement
action is necessary
to
ensure
compliance with NRC regulatory requirements.
