1 NRC BULLETIN 88-02, "RAPIDLY PROPAGATING FATIGUE CRACKS IN STEAM GENERATOR TUBES" (TAC NO. M67301)
On February 5,
- 1988, the staff issued NRC Bulletin 88-02, "Rapidly Propagating Fatigue Cracks in Steam Generator Tubes."
The Bulletin requested that holders of operating licenses or construction permits for Westinghouse (W) designed nuclear power reactors, with steam generators having carbon steel support
- plates, implement action to minimize the potential for a steam generator tube rupture event caused by a rapidly propagating fatigue crack such as the one that occurred at North Anna Unit 1 on July 15, 1987.
By letters dated October 16,
- 1992, and January 28,
- 1994, you responded to our letter and safety evaluation dated August 10, 1992.
In the first letter you addressed the Primary-to-Secondary Leak Rate Monitoring Program required by Action C. 1 of the Bulletin.
In the second letter you provided the results of the analysis done by Westinghouse for tube vibration-induced fatigue, and the corrective actions and monitoring to be performed for the tubes identified as susceptible.
Please advise us, in writing, when you have removed tube R8C35 from service.
4 The staff concludes that your actions resolve the issues identified in Bulletin 88-02 and are, therefore, acceptable.
This finding is subject to the development of administrative controls by you to ensure that updated stress ratio and fatigue usage calculations are performed in the event of any significant changes to the steam generator operating parameters (e.g.,
steam pressure and flow, circulation ratio) relative to those assumed in the WCAP analysis.
Implementation of the additional administrative controls may be subject to verification by an NRC staff inspection.
If, for any reason, you do not plan to implement such administrative controls, 'please advise within 30 days of receipt of this letter.
