Thank you for your 'letters dated February 1 and Narch 12 and 14, 1991, in responSe to our December 27, 1990, letter and subsequent conversations concerning allegations pertaining to (1) an improperly installed anchor bolt, (2) a defective field weld, (3) use of an incorrect bolting and welding procedure, and (4) a wrong pipe being cut during a "tie in" at the D.
C.
Cook plant (Allegation RIII 90-A-0132).
In our letter; vie requested that you investigate these matters and submit the results of your investigation to the NRC.
Your response indicated that concern (1) was substantiated and an evaluation was performed which determined that an operability concern did not exist and appropriate disciplinary actions were taken to ensure the individuals involved follow and adhere to plant procedures.
Concerns (2) and (3) could not be substantiated.
Concern (4) could be neither confirmed nor denied.
Through our= review and onsite inspection, we found your investigation to be sufficient to support your conclusions and have no additional questions regarding this matter.
We request that you retain the supporting documentation for your investigation of this allegation for a minimum of one year from the date.of this letter.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter, wi 11 be placed in the NRC Public Document Room.
