A Fitness-For-Duty (FFD) inspection is scheduled to be conducted at D.

C.

Cook Nuclear Power Plant the week of February 19-22, 1991.

Hr. J.

R. Kniceley of this office has been designated as the primary contact for this inspection and can be contacted at telephone number (708) 790-5641.

As part of the inspection

process, FFD written poli'cies and procedures will be reviewed in the NRC Region III office prior to the onsite inspection.

You are requested to provide us a copy of the procedures and policies pertaining to your FFD program.

These documents will be returned to you, or destroyed if requested, after the inspection report has been prepared.

Any of the documents that are considered as exempt from public disclosure in accordance with 10 CFR 2.790 should be identified as such and a statement of the reasons for nondisclosure should also be provided as required by 10 CFR 2.790(b).

The FFD policies and procedures should be provided.to us by February 4, 1991.

FFD training material will be reviewed onsite and does not have to be sent to us.

During the inspection, interviews will be conducted with the onsite FFD

manager, the Hedical Review Officer, the Employee Assistance Program
manager, and personnel responsible for the onsite collection and, if appropriate, onsite testing facility. If feasible, please contact Hr. J.

R. Kniceley by February 11, 1991, to arrange an interview schedule for the above personnel.

Interviews of randomly selected licensee employees and contractors will also be conducted during the onsite inspection.

A room to conduct the interviews will be required.

FFD program audits and program records (including training records, personnel records for FFD program personnel, records of pre-access authorization inqui ries, executed contracts with contractors/vendors, and records of chemical test results and appeals, etc) should be available for review. If any of these records are located offsite, please inform Hr. J.

R. Knic'eley as soon as practical.
