Dear Mr. Davis:
The purpose of this letter is to respond to your staff's request to provide the follow-up countermeasures we intend to take as a
result of an inappropriate inverter breaker switch setting discovered on April 10,- 1989.
The attached countermeasures will also be included in our notification S-LER-89-1, which is being submitted under 10 CFR 73.71.
This document has been prepared following Corporate procedures that incorporate a reasonable set of controls to ensure its accuracy and completeness prior to signature by the undersigned.

Continue increased security patrols throughout the plant during the 1989 Unit 1 refueling outage.
Distribute an awareness letter to all site personnel emphasizing increased alertness for mis-positioned switches or valves and the possibility of persons treating equipment or systems in an unacceptable manner.

The Security Manager will coordinate with Assistant Plant Managers to assure dedicated technical expertise is availab e to assist Security in the event of future occurrences involving plant equipment or systems.

Senior plant management will closely monitor Unit 1 restart ac vti ities and evaluate problems encountered during evolutions.
They will notify Security immediately if evidence of unexplai lained events is detected.
The plant Security Manager will ensure that plant Security Staff personnel are on call continuously during restart activities.
