SUBJECT'D. C. COOK INSPECTION REPORT 50-315/99015(DRP); 50-316/99015(DRP)

Dear Mr. Powers:

On July 16, 1999, the NRC completed an inspection at your D. C. Cook Units 1 and 2 reactor

facilities. The inspection was an examination of activities conducted under your license as they

relate to compliance with the Commission rules and regulations and with the conditions of your

license. Areas reviewed Included Operations, Maintenance, Engineering, and Plant Support.

Within these areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observations of activmes in progress.

The enclosed report presents the results ofthat inspection.

Your staff successfully restored three offour diesel generators to an operable condition along

with the associated train of boration control resulting in an increased abilityto address

postulated reactor coolant dilution events.

The fourth diesel generator had been restored to an

operable condition and discussed in a previous inspection report. This marked the first time

since January 1999, that all four diesel generators were operable.

We also noted that your

staff Identified in a timely manner and took aggressive corrective action to address an adverse

trend in configuration control and attention to detail.

Even though previous escalated enforcement action had been issued for ineffective control of

contractors, an example ofweak control of contractors was identified by the inspectors.

The

example involved oversight of maintenance work on the Unit 1 West Component Cooling Water

(CCW) heat exchanger.

The inspectors'lso Identified an organization-wide misunderstanding

ofthe use of your Quality Assurance Program and how the program interfaces with contractor

quality assurance

programs.

In response to the inspectors'oncern,

your staff took prompt

and appropriate corrective actions.

Corrective actions included the holding of a plant-wide

stand down to emphasize the Cook Quality Assurance Program and the need for contractor

control..

During the inspectors review of the control of contractors, two violations of NRC requirements

were identified. The first violation, identified by the NRC inspectors, was a failure to followthe

Quality Assurance Program. The procedure used by the contractors when pulling tubes from

the Unit 1 West CCW heat exchanger was not approved at the appropriate management

level.

The second violation, identified by the inspectors, involved the failure of the contractors to

followtheir procedure while removing tubes from the Unit 1 West CCW heat exchanger.

These Severity Level IVviolations are being treated as Non-Cited Violations (NCVs).

Appendix C of the Enforcement Policy requires that for Severity Level IVviolations to be

dispositioned as NCVs, they be appropriately placed in the licensee's corrective action program.

Implicitin that requirement is that the corrective action program be fullyacceptable.

The plant

corrective action program was not adequate

and has been the focus of significant attention by

your staff to improve the program.

While your staff and the NRC have not yet concluded that

the corrective action program is fullyeffective, the corrective action program improvement

efforts are underway and captured in the Restart Plan which is under the formal oversight of the

NRC through the NRC Manual Chapter 0350 Process, "Staff Guidelines for Restart Approval."

Consequently, these issues will be dispositioned as NCVs.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, its

enclosure, and your response, ifyou choose to provide one, will be placed in the NRC Public

Document Room.
